SUBJECT: HRC INSPECTION REPORT NO. 50-528/529/530/92-41

Thi:, refers to the inspection conducted by J. Sloan,11. Freeman, D. Kirsch, F.

Ringwald, and T. Sundsmo of this office, on November 3 through December 14,

1992.

The inspection included a review of activities authorized for your Palo

Verde facility.

At the conclusion of the inspection, the findings were

discussed with thnse members of your staff identifit,d in the enclosed t eport.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective exaninations of procedures

and representative records, interviews with personnel, and observation af-

.

activities in progress.

Your attention is directed to paragraph 7 of the enclosed report which

discusses a reactor trip at Unit-1 caused by the failure-of a protective relay

associated with the main generator.

This event is of concern in that vendor

iMormation was previously available indicating that the relay which failed

should have been modified to correct known problems. Additionally, it

appeared that the relay vendor was contacted.only after NRC questioning

prompted the action.

You are encouraged to ensure that vendo,' information is

adequately controlled and assessed.

Two non-cited violations were identified during this- inspection. The first

dealt with a technician performing an independent verification of a valys

'

position in a tanner that did not meet your expectations.

The second ins *,ance

involved the rem 9 val of a circuit breaker from its cuo'cle without a

maintenance ~ order ta control the work.

Both violations were not cited because

of your corrective actions and the relatively low safety significance of the

occurrences.
