(i).

Pipe Clamps BE-412, 413, 418, 419 & 421 N-1 (3/4 and 1" size) and Pipe Clamps BE-412, 413, 418, 419 & 421 N-2 (l" size) supplied to numerous facilities. List is being cmpleted for notification.
(iii).
Clamps supplied to Pacific Scientific by Basic Engineers Division of NAVCO. (See letters attached) l (iv).
Improper installation may cause pipe clamp to slip l
circumferentially. This slippage may occur when a cmpressive load less than the faulted rated load is applied to the clamp at an angle of 5 to centerline of the clamp as permitted by the clamp design drawing.
(v).
First notification of a potential defect was received by the writer by certified mail on 6-13-85 at which time NRC was notified.
(vi).
Pacific Scientific is in the process of cmpiling a list-ing of recipients and locaticns to which suspect pipe clamps were shipped.

Proble has been evaluated by Basic Engineers, the pipe Clarnp manufacturers, and results are stated in the attached letter.
Corrective action is being undertaken at this time. No ccrapletion date has been established.

Cleveland Electric Illtunnating Co. has reported the -
potential proble to the NRC under Federal Regulation 10 CFR 50.55e.(see letter attached).

Basic Engineers Division of Nati:ral Valve and Manufacturing Cmpany has W aware of conditions in the installation of its E-412,413,418,419 & 421 pipe clanps which may cause them to slip ciretznferentially around the pipe. This slippage may occur when a cmpressive load less than the faulted rated IT.d (Service Ievel D) is applied to the clanp at an angle of 5' to the centerline of the clang as permitted by the clanp design drawing.
The problem was discovered cri April 16, 1985 at Cleveland Electric Illuminating Cm pany's Perry Nuclear Station, Unit #1. Basic Engineers innediately began a series of analytical evaluations and service simulation tests. During the tests, a cmpressive load equal to the faulted rated load (Service Ievel D) was applied to the clanp at an angle of 5' frmi the clanp centerline. The installaticm was varied by torquing the bolts of the clang to different values. 'Ihe results of the evaluations and tests show that a potential problem exists with the E-412,413,418,419&421N-13/4" and la pipe size clanps and with the E-412,413,418,419 &
421N-21" pipe size clanps installed with bolt torque values less than 175 in.-lbs. and 360 in.- Bs. respectively.
Basic Engineers will be revising the clang load r apar ity data sheets and/or Installation Instructirms to show the following:
- 1) E-412,413,418,419&421N-1 (both 3/4" & 1" pipe size clanps) a) '1he two SA-307 Gr.B bolts will be changed to SA-193 Gr.B7 bolts or studs of the same size with SA-194 Gr.2H Hex Nuts. Hardened steel washers shall be used.
b) '1he bolts shall be torqued to 175 inch-lbs. I O in.-Ibs.
'Ihe torque should be rechecked after 15 minutes.
c) 'Ihe maxinun back to back dimension at the spacer shall be 0.575 inches.

E-412,413,418,419&421N-2 (l" pipe size clanps) a) 'Ibrque the two SA-307 Gr.B bolts to 360 inch-lbs. I 1 in.-lbs.

Cleveland Electric Illuminating Company (CEI) has received and installed at Perry Nuclear Station Unit 1,, Pacific Scientific Kin-Tech Division mechanical shock arrestor assemblies that include Basic Engineers (BE)-
BE-419N pipe clamps.
On April 16, 1985 CEI infomed BE that it had reason to believe that some of these clamps when installed in a compressive load situation with bolts torqued in accordance with Pacific Scientific's in-stallation instructions would slip circumferentially around the pipe when a thrust less than Pacific Scientific's rated faulted load is applied at o
an angle of 5 to the centerline of the clamp, as permitted by the clamp design drawing.
In your May 22, 1985 telephone conversation with BE's Paul Scholar, you requested that CEI be provided by May 30, 1985 answers to four questions.
CEI's questions and BE's answers are as follows:
1.
Question -
What is the status of Basic Engineers investigation of the problem described above?
Answer -
The investigation is complete except to confim that the clamps or bolts do not relax or defom over time thus requiring additional torquing of the nuts.
2.
Question -
What is Basic Engineers currently doing about the problem?
Answer -
Long tem testing at 175 inch-pound and 360 inch-pound torque has j
begun. Torques will be checked at one month intervals for six months.

The problem is limited to the following clamp figure numbers and sizes an1 is as described below.
BE 419N-1:
3/4" and 1" pipe size clamps under a 5 angular compressive load slip at less than Pacific Scientific's rated faulted load when installed with bolts torqued as specified in Pacific Scientific's installation instructions.
BE 419N-2:
1" pipe size clamps under a 5 angular compres-siveload slip at less than Pacific Scientific's rated faulted load when installed with bolts torqued as specified in Pacific Scientific's installation instructions.

Question -
What does Basic Engineers recommend to solve the problem?
Answer -
BE 419N-1 (Both 3/4" and 1" pipe size clamps) a.
Replace the two SA 307 Gr. B bolts furnished with the pipe clamp with SA 193 Gr. B7 bolts of the same size including hardened washers b.
Torque bolts to 175 inch-pound 110 inch-pounds.
Torque should be checked after 15 minutes.
c.
If, after bolts have been torqued as specified, tension is removed from the bolts, they should be replaced with new bolts and torqued as specified above.
d.
Maximum clamp back to back dimension at the spacer must be 0.575 inches.

Pagm 3 May 30, 1985 The determination of those clamp sizes subject to the problem and the con-finnation of the adequacy of the solutions were the result of physical testing. Compressive loads gere applied thru a sway strut to installed pipe clamps at an angle of 5 to the centerline of the pipe clamps.
