NORTHERN STATES POWER COMPANY (MONTICELLO)
Problem:
CRACKS IN 4" RECIRCULATION PIPING The licensec informed Region III by telephone on January 21, 1975, that ultransonic inspection of the 4" stainless steel bypass lines around the primary recirculation pump discharCe valves had been performed in accordance with R0 Bulletin No.74-10A dated December 17, 1974.
Indicatiotis of circumferential cracks were found in the heat affected :one of three welds upstream of the bypass valve.
Two of the indications were identified in Loop A, one in Loop B.
The three defects are similar to those identified in Dresden 2 and Quad Cities 2 described in DRO Notification of an Incident or Occurrence No.
137 dated December 17, 1974, and No. 139 dated December 24, 1974, respectively.
Operations to date at Monticello have been conducted with the bypass valves open while the bypa.s valve at Dresden and Quad Cities and other operating BWR's have, until recently, been closed during operations.

1.
The unit was shut down on January 9, 1975, for a planned refueling outage. While shut down, the inspection requested in R0 Bulletin 74-10A was performed.
2.
U1 transonic testing revealed the following indications:
a.
Two indications in Loop A of isolable piping-to-elbow shop welds upstream of the bypass valve.
The indications are on the pipe side of the velds in the heat affected zone.

b.
One indication in Loop B at an isolable piping-to-elbow shop weld upstream of the bypass valve.
The indication is on the pipe side or the veld in the heat affected zone.
(Attachment 2).

These areas had been ultransonically tested in November 1974; no unacceptable indications were detected at that time. A comparison of signal traces from the November 1974 examination with the current examination disclosed an apparent growth from acceptable non-linear indications to linear indications up to 1/4 of the pipe circumference with crack depths up to 1/4 to 1/2 of the pipe wall thickness.

The licensee has confirmed the indications by radiography and is currently reviewing corrective action.

RO III follovup is continuing and further R0 action is contingent on additional findings and corrective action to be taken by the licensee.
