RHR PUMP MCrr0R FAILURE The enclosed Inquiry Report from our Region III (Chicago)- Office is forwarded for your information.

The report relates to the failure of a motor which drives one of the four residual heat removal pumps and cracking of air diffusers in the motors for tuo other RHR pumps. A preliminary copy of the enclosure was prcvided earlier to Jim Shua.

Description of Particular Event or Circumstance:

The motor which drives one of the four Residual Heat Removal (RER) pumps failed on December 16 during operation in the shutdown ecoling mode. Initici investigation determined that the sheet metal air diffuser in the motor hcd failed from fatigue cracking originating near the anchor bolt holes. Fragments of the air diffuser cau:cd shorting of the motor windings.

Inspection of the remaining RER motors disclosed two additional air diffusers with cracks initiated.

Severity of the cracks was proportional to the operating hours (177 to 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br />) on the motors.

Telephone call from G. Jacobson, Plant Results Engineer, to RO:III on October 12, 1972.

Description of Particular Event or Circumstance:

During a routine surveillance testing of the standby liquid control pumps on October 11, 1972, the No. 12 pump f ailed to develop the re-quired system head of 1275 psig on one of two tries. On October 12, 1972, the same pump failed on one out of 4 tests.

In both cases the discharge pressure was 900 psig.

The licensee suspects that one of.

the piston's check valves maybe hanging up causing the loss in dis-charge pressure.

Action by Licensee:

Ihe licensee has declared the pump inoperabic and is getting ready to disassemble it.

Pump No. 11 has been tested satisfactorily as required by Technical Specification paragraph 4.4.B.

The licensee plans to report the matter to Licensing within 30 days.
