- Office of Nuclear Reactor Regulation items of interest Week Ending January 22, 1993 fitroatrick following an extended refueling outage that began on 11/27/91, the licensee commenced startup of the facility on 01/02/93. The reactor reached criticality on 01/03/93 at 1:50 a.m. Currently, extensive Reactor Core Isolaticn Cooling and High Pressure Injection Cooling system testing is being l performed and reactor power is being held at 21% of rated power. The current l Schedule calls for synchronizing the generator to the grid on 01/23/93.

An Individual Plant Examination (IPE) review team (NRR/RES) will visit the i fitzpatrick plant on 01/27-29/93-to obtain information from the licensee and  !

walk down selected plant systems in order to facilitate preparation of a staff I evaluation of the Fitzpatrick IPE.

i On 02/01-05/93, Region 1 will conduct a team inspection of the Fitzpatrick motor-operated valve (MOV) program (Generic Letter 89-10).  ;

Brunswick Units 1 and 2 Repair of Emergency Diesel Generator EDG-1 has been completed, and a 24-hour -

endurance run began on January 20, 1993. EDG-1 repairs included the straightening of the crankshaft, replacement of two damaged main bearings and i an idler gear, and rework of the flexible drive for the lube oil pump and jacket cooling water pump. The licensee will submit a report on the failure analysit, along with a fatigue analysis on operation with the rcpaired crankshaft. The staff will review the analysis before further discussions I

with the licensee on additional diesel testing. >

Inspection of the other EDGs for possible similar problems as EDG-1 is in progress. The inspection of EDG-4 has been completed satisfactorily, and surveillance testing was conducted on January 19, ~.993.

I Interim SG Tube Pluaaina Criteria for Summer and Kewaunee (Ed 2).

During the week of January 11, 1993, the NRC staff met with representatives from both the V.C. Summer Nuclear Station and the Kewaunee Nuclear Power Plant to discuss proposals to implement an alternate steam generator tube plugging criteria. The proposals for both licensees involve the use of a 1.0 volt bobbin voltage-based alternate plugging criteria.

- The proposals are similar to the previously approved voltage-based tube repair criteria implemented at Trojan, Farley Units 1 and 2, D.C. Cook Unit 1, and >

Catawba Unit 1. However, the methodology used to calculate the expected primary-to-secondary leakage differs from that used in the previously approved submittals. These proposals are being evaluated by the staff. ,

On January 12-13, 1993, emergency preparedness staff from Headquarters and NRC Regions met in Region 11 offices in Atlanta. The meeting was also attended by representatives from AE00's Emergency Response Branch, NRC's Office of Enforcement, and NRR's Operator Licensing Branch. Presentations and discussions covered topics of current interest ranging from Hurricane Andrew to rules, policies, and procedures governing NRC licensing and inspection in the emergency preparedness area.

ligetino with Southern California Edison (SCO to Discuss Procurement DJ.dication issues On January 13, 1992, the Director, Division of Reactor Inspection and Licensee Performance, and members of the Vendor Inspection Branch met with SCE representatives at the San Onofre Nuclear Generating Station (SONGS) to discuss key procurement and commercial grade dedication issues identified during the five pilot inspections and to discuss certain dedication packages from the December 1991 procurement inspection at SONGS.

Prior to the start of the meeting, the NRC representatives toured Units 2 and

, 3 of SONGS with the NRC Senior Resident Inspector. SCE then conducted a tour of their new Test Laboratory and_ the warehouse at SONGS, including a demonstration of the seismic shake table. The capabilities of this testing laboratory are quite extensive, and once fully operable, should greatly

, enhance the program for the dedication of commercial grade it::ms at SONGS.

The dedication issues discussed were those where differences between NUMARC and the NRC positions were previously identified. SCE senior management stated that they had no disagreements with the staff positions on the specific dedication issues and stated that the positions appeared to define methods ,

which would need to be followed to conform with 10 CFR 50, Appendix B requirements. SCE also stated that their current dedication program conformed 91-05. However, SCE did identify a few issues, with including NRCsampling, Generictraceabi letter (GL)lity, and like-for-like replacements, where they felt some clarification would more precisely identify the staff's expectations. .

During discussions or, specific dedication packages from the SONGS inspection, SCE presented positive corrective actions taken since the 1991 inspection.

SCE also went over dedication packages for items such as molded case circuit breakers, lubricating oil, and packing rings, completed in 1992 using their-current program requirements for dedicating commercial grade items. Both the corrective actions and the 1992 dedications were consistent with staff's positions documented in GL 91-05 and the list of dedication issues, as well as c Appendix 0 requirements.

3 Sirewell B Control Room Interfaces From January 12-14, 1993, James Bongarra of the Human factors Assessment Branch (HHFB), met with Mr. David Whitfield, HM Principal Inspector, Nuclear Installations Ins)ectorate (NII) and members of Nuclear Electric (NE) in London and Ipswic1, England. Topics of discussion with Nil included the methodology and acceptance criteria being used by NE to validate the human factors engineering (HFE) design of the Sizewell B main control room and auxiliary shutdown room (ASR) and the HFE program review model developed by HHFB for evaluating human systems interfaces (HSI) for advanced reactors.

On Wednesday, January 13, Mr. Bongarra spent the day with Mr. Whitfield and several members of NE at the Sizewell B simulator facility in Ipswich, England, observing simulator trials which were focused at exercising various operating and emergency procedures and assessing the HSI characteristics of the main control room (MCR). Discussions were conducted with the NE staff about the features of the ASR which exists below the MCR simulator as a separate full scope, full scale simulator. Jim also had discussions with NE personnel concerning the plant overview panel, a combination analog-digital control and display panel unique to the Sizewell B MCR design which provides additional safety monitoring and shutdown capability in the event of a loss of main control room instrumentation and controls.

Licensed Operator Re. qualification proaram Evaluation at South Texas During the week of January 11, 1993, Brian Hughes of the Operator Licensing Branch cnd Mary Ann Blamonte of the Human factors Assessment Branch observed a '

Licensed Operator Requalification Program Evaluation conducted at South Texas by members of the Region IV operator licensing staff. This was the first of eight evaluations to be performed by all the regions using the newly issued Temporary Instruction 2515/117, " Licensed Operator Requalification Program Evaluation." Vpon completion of the eight evaluations, the Operator Licensing Branch will issue a written summary of the inspection experiences and any recommendations for improving the inspection guidance. Ms. Blamonte monitored the inspection activities to identify their relationship to inspection activities proposed for training inspections resulting from the new training rule when implemented.

Office of Nuclear Material Safety and Safeguards items of Interest Week Ending January 22, 1993 Quality Assurance Insntction On January 12-14, 1993, the Transportation Branch staff conducted an inspection at the Department of Energy's Waste Isolation Pilot Plant (WIPP) in Carlsbad, New Mexico. The inspection team reviewed implementation of the quality assurance program in the procurement, fabrication, maintenance and use of Nuclear Regulatory Commission-certified Model No. TRUPACT 11 containers.

The WIPP Land Withdrawal Act rcquires that containers used to or from WIPP be determined by NRC to satisfy its quality assurance requirements. Preliminary results were discussed at_the exit meeting. There were no nonconformances or violations noted during the inspection.

Interanency_Bettina with Jaojn On January 13, 1993, an International Safeguards Branch staff member participated in a U.S. Government interagency meeting with Japanese representatives to discuss proposed changes to the Material Categorization Table in the International Atomic Energy Agency's INFCIRC/225/Rev. 2, "The Physical Protection of Nuclear Material." A significant change proposed by the Japanese would entail inclusion of waste forms in the Categorization T abl e . In connection with the discussion on physical protection at nuclear f acilities, a visit was arranged to the Calvert Clif fs nuclear power plant for 2 the Japanese and a Department of Energy staff member on January 14, 1993, to observe the facility's physical protection system.

Utg_Jork State Conference on low-level Radioactive Waste Manaaement and Storage Division of Low-level Waste Management and Decommissioning (LLWM) staff -

participated in a New York State meeting on low-level waste (LLW) storage on January 14, 1993, in Suffern, New York. The meeting was designed to bring together representatives of government, industry, and other organizations to discuss and share information on the management and storage of LLW. LLWM staff participated in a panc! discussion with representatives from the various State of New York licensing organizations involved with the Agreement State program. The LLWM participants presented the agency's overall position on LLW storage and discussed the provisions of the proposed rulemaking for post 1995 storage.

During January 11-15, 1993, quality assurance (QA) staff of the Division of

!iigh-level Waste Management (HlWM) observed a QA performance-based audit of EM-343 performed by the DOE Office of Civilian Radioactive Waste Management (OCRWM) QA Division in Germantown, Maryland. The State of Nevada was notified but elected not to participate in the observation of this audit. The performance-based aspects of the audit included the evaluation of the processes of the EM-343 Technical Review Group, Operational Readiness Review, JANUARY 22, 1993 ENCLOSURE B

2 verification methodology (audits /surveillances), program execution documents, and eleven of the QA program elements to determine whether the EM-343 QA program meets the requirements and commitments imposed by the 00E/0CRWM.

The HLWM observers determined that the audit was effective, and implementatior of the EM-343 QA program was adequate in nine of fifteen areas audited, with the remaining determined to be marginally effective. Four preliminary Corrective Action Requests (CARS) were issued by the DOE audit team in the areas of technical review operational readiness review, auditing, and QA program control. Thedeficieu ~s identified in the CARS are not significant in terms of the overall EM-343 QA program, Surveillance of the U.S. Geoloaical Survey (USGil Division of High-Level Waste Management (HLWM) staff observed a Department of Energy, Office of Civilian Radioactive Waste Management, Yucca Mountain Quality Assurance Division (YMQAD) quality assurance surveillance of the USGS January 11-15, 1993, in Denver, Colorado. The surveillance evaluated the adequacy and effectiveness of the USGS Yucca Mountain Project investigation and corrective actions for a deficiency involving failure to follow procedures in field data gathering activities. The surveillance team determined that the USGS corrective action was adequate, and the HLWM staff agrees with this ,

assessment.

Final preparations were made for the incident Investigation Team (IIT)  ;

The training includes both a. reactor and a nonreactor scenario and will involve both senior NRC and industry management.

Incident Response Branch (IRB)

On January 9-10, Branch supervisor: held a retreat at Hunt Valley, Maryland.

The focus of the meeting was Branch organization and task prioritization given -

the cancellation of the Federal Field Exercise-3.

On January 12-13, IRB staff participated in a Regional Emergency Preparedness counterpart meeting in Atlanta, Georgia. The meeting was useful in integrating NRR and AE00 activities in emergency planning and response l activities.

On January 14, Branch staff participated in a meeting of the Prevention Committee of the National Response Team in Washington, D.C. The meeting was  ;

the first for a newly formed group whose purpose is to integrate information concerning federal emergency planning activities.

forms Automation The Office of Information Resources Management and the Forms Automation Committee are currently examining software packages for forms automation. A forms management system that does not utilize graphics traveling through the local Area Network (LAN) must be used. A software package has been purchased that stores graphics images and fonts, in a cartridge, at each printer location. Software then imports the graphics image to the user work station and back to the printer without any fonts traveling through the LAN. The first 25 NRC forms have been put into the software and a one month pilot test will be conducted by *.he IRM forms analyst.

Training Command in Dam Neck, Virginia. The Command uses a videoconferencing  ;

system to provide training to troops in Newport, Norfolk, Charleston, and Mayport. OP is evaluating the use and requirements for videoconferenced training, and is working with IRM to determine feasibility for NRC.

Employee Assistance Proaram Workshop Attended On January 21, Pat Kaplan attended a workshop presented by the D.C. Chapter of .

the Employee Assistance Program Association on " State-of-the-Art Detoxification and Rehabilitation Procedures." The program focused on the management of addict on detoxification in an outpatient setting and a new medication that mir .aizes withdrawal symptoms.

Sianificant Enforcement Actions

1. A Notice of Violation and Proposed Imposition of Civil Penalties in the l amount of $3/,500 was issued on January 19, 1993 to Nuclear fuel  ;

Services, Inc., Erwin, Tennessee. The action was based on two separate events. The first involved a fire and explosion in the High Enriched  !

Uranium Recovery facility on September 10, 1992. The failure to follow i t applicable process control procedures created a substantial potential i for serious personal injury and possible radionuclide uptake had personnel been in the immediate vicinity. Therefore, this violation has bon categorized at Severity Level 11 and assessed a $25,000 civil penalty. The civil penalty was mitigated 25% for licensee i identification of the root cause, and an additional 50% for corrective

] action. However, the civil penalty was escalated 100% for poor past i performance and prior opportunity to identify the problem. (EN 93-003) l

2. A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 was issued on January 21, 1993 to the Carolina Power and Light Company (Brunswick Unit ?). The action was based on two -

violations that resulted in a significant radioactive contamination i event at the plant and they have together been classified as a Severity '

! Level 111 problem. Specifically, the licensee failed to label a source l or maintain a comprehensive inventory of the sealed sources onsite and ,

failed to adequately evaluate the potential radioactive hazards prior to  !

cutting a holder for a startup source that was thought to be empty.

2 Consequently, the 5.4 Curie americium-241 source that was actually j contained in the holder caused substantial alpha contamination of the fuel building and the surrounding areas prior to the problem being detected, contained, and cleaned up. Consideration _was given to

- mitigating the base civil penalty for the licensee's identification of

, the contamination problem as well as for the corrective actions taken.

However, the licensee could have prevented this problem and also could ,

have detected the contamination event earlier. Therefore, in order to emphasize the significance of this event, discretion is being exercised in accordance with section Vll.A.1 of the Enforcement Policy and a civil penalty equal to the base amount of $50,000 was proposed. (EN 93-004)

3. An Order Suspending License (Effective Immediately) was issued on '

January 20, 1993 for licensee facilities at Oncology Service Corporation, State College,_ Pennsylvania. The action was being taken pending the completion of ongoing NRC-investigations and. licensee .

corrective actions following a November 16, 1992 incident in-which aa '

iridium-192 source broke off of the metal drive wire of a high dose rate remote afterloader brachytherapy machine and remained lodged in a i catheter that had been placed in a patient to accommodate the treatment.

As a result-of this incident, a subsequent NRC inspection, and the NRC  ;

-staff's review of internal licensee correspondence, the staff has determined that there exists a- breakdown -in the control of licensed activities at the-corporate level wherein key licensee employees at (

several authorized locations of use do not know the requirements of the  !

NRC license and have not been adequately trained in pertinent regulatory l requirements and radiation protection practices. The staff has concluded that the licensee's radiation safety officer is not willing to  !

supervise the radiation safety program on a routine basis at the various  !

locations of use listed on the license. (EN 93-008) {

, 4. A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $125 was issued on January 22, 1993, to G. R. Osterland

. Company, Cleveland, Ohio. The action was based on_ the licensee's

- failure to maintain control of licensed material when a soil moisture density gauge was damaged by a road paving machine (roller)'after the authorized user left it unattended briefly while he went to talk to an engineer at the paving site. The $500 base civil penalty was mitigated 25 percent because the licensee demonstrated initiative in identifying the root cause of the self-disclosing event, it was also mitigated 50 percent for effective corrective action. (EN 93-005)

. 5. A Notice of Violation and Proposed imposition of Civil Penalty in the amount of $1,000 was issued on January 22, 1993, to American.Cyanamld Company, Princeton, New Jersey. The action was based on seven "

violations that, in the aggregate, represent a breakdown in the control and oversight of the radiation safety program. The violations have been categorized in the aggregate as a Severity Level 111 problem. The base civil penalty amount for a Severity Level 111 problem is $500. The base civil penalty was escalated by 50% because the NRC identified the violations, and escalated by $100% based on the licensee's past enforcement history. The penalty was also mitigated by 50% based on the licensee's prompt and comprehensive corrective actions. Therefore, on -t balance, the base civil penalty amount was escalated by 100%. (EN 93- +

6. A Notice of Violation and Proposed Imposition of Civil Penalty in-the amount of $3,750 was issued on January 22, 1993, to Pike Community '

Hospital, Waverly,' Ohio. The action was based on a significant-breakdown in the control of licensed activities involving 16 violations.

The $2,500 base civil penalty was escalated 50 percent for NRC identification. (EN93-007)

Civil Penalties Paid ,

1. South Dakota Department of Transportation (SD 00T) paid the civil penalty in the amount of $3,400. The action was based on several violations identified during an investigation and inspection of the SD-D01_ facilities in Brookings and Pierre, South Dakota, regarding the-reported theft of a moisture / density gauge. The specific violations -
involve: (1) the failure to maintain constant surveillance of licensed material; (2) the failure to provide complete and accurate information to the NRC; (3) the failure to meet a number'of requirements related to the transportation of radioactive material, and (4) six failures to meet ,

the administrative requirements contained in NRC regulations. The -

resulted in a significant potential for inadvertent radiation exposure to a member of the public and (2) the 50 DOT employee's actions (leaving the device unsecured and unattended for 1.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br /> in a public parking lot) showed a careless disregard for the NRC requirements that are designed to prevent the loss of such material. In addition, the willful failure on the part of two individuals to provide complete information to the RSO and to ensure that the information provided to NRC was complete and accurate was classified as a Severity Level 11 violation.

The violations associated with the transport of the gauge indicate a potentially significant lack of attention toward licensed responsibilities and were classified collectively as a Severity Level 111 problem. (EA 92-150)

2. Washington Public Power Supply System (WNP-2) paid the civil penalty in i the amount of $75,000. The action was based on three violations identified during the review of the August 15, 1992, reactor power oscillation event which resulted in the manual scram of the reactor.

These violations were collectively classified as a Severity Level 111 problem. The base civil penalty of $50,000 was been escalated 50 percent after finding 50 percent escalation appropriate for both past poor performance and prior opportunity to identify the problems and 50  !

percent mitigation for the licensee's corrective actions. (EA 92-200)

3. R. S. Scott Associates, Alpena, Michigan, paid the civil penalty in the amount of $250. The action was based on the licensee's failure to maintain control of licensed material when a soil moisture density gauge +

was moved after the authorized user left the gauge unattended. An individual moved the gauge away from an excavation in-order to operate heavy equipment and repositioned it at a safer location at the construction site. The individual who moved the gauge was not an employee of the licensee or an authorized _ user. The $500 base civil penalty was escalated 50 percent for NRC identification and mitigated 100 percent for the licensee's good past performance. (EA 92-236)

During the week, the installation of waterproof f.:embrane for the TWTN plaza area in front of the building was near completion. The gazebo roof was framed out and covered with tar paper. flashing for the skylight was installed.

Work continued on framing and pouring concrete for various steps in the plaza area. Caulking above windows on the exterior of the south side of the building began during the week. All windows are now installed except for narrow openings on each of floors 2-8 for access.

The main electrical bus duct is installed from the Level B Electrical Room through the floor 6 slab. Electrical systems installation activity increased significantly during the week in penthouse machine rooms and mechanical rooms on floors 2-6.

Marble is installed up to the 16-foot level in Plaze level elevator lobby and on one main lobby wall. Installers laid out and began matching the verde (green) marbic for installation on the eccentric '411 in the main lobby. l On January 21, at the NRC/GSA/Lerner progress meeting at GSA, the Director requested that GSA research and evaluate the possibility of completely re-surfactng the uncovered portion of the OWFN plaza. deck between the NuReg Cafe and the TWEN plaza deck rather than merely patching and repairing the existing membrane. Leaks within the garage of OWFN are attributed, in part, to damage and deterioration of this waterproof membrane. The existing contract between the Government and the developer calls for " removal and replacement of a portion of the waterproof covering of the 0WFN plaza" but Lerner's i representative said that Sigal was planning only to repair damage to the l existing membrane that it caused or discovered during the uncovering.

On January 22, Office staff met with representatives of Lerner, Sigal and GSA to review the current status and future disposition of all pending TWFN change orders.

definitions e.g. validation. This framework may be useful to the NRC in its  ;

site decontamination work. Mr. M. Nalluswami of HRC's decommissioning staff also attended the meeting. Ms. Melley expressed an interest in meeting with appropriate NRC staff and Ms. Lopez-Otin will pursue this matter. Copies of .

National Indian Nuclear Waste Policy Committgg Rosetta Virgilio, State Programs, and Francis Cameron, Office of the General Counsel, participated in a meeting of the National Indian Nuclear Waste Policy Committec (NINWPC) on January 19, 1993 in Crystal City, Virginia. The i NINWPC, which serves as a tribal advisory group to Department of Energy's (00E) Office of Civilian Radioactive Waste Management, met during the National Congress of American Indians (NCAl) Executive Council Meeting to discuss nuclear waste issues that would impact Indian lands. Topics of discussion included NRC's enhanced participatory rulemaking for decommissioning; the signing of a five-year agreement with DOE (December, 1992) to continue an education and communication project on DOE's tribal initiatives; and DOE's strategy for management of commercial spent fuel, environmental restoration and Indian Policy implementation. A representative of the Skull Valley _

Goshute also presented a video on its MRS feasibility study.

Michiaan Air Pollution Control Comminig.n Meetino Roland Lickus, Regior.111 State Liaison Officer, George M. McCann, Region Ill Nuclear Materials Licensing Section Chief, and Meg Harvey, Environmental Scientist, Office of Nuclear Material Safety and Safeguards, attended a meeting of the Michigan Air Pollution Control Commission (MAPCC) on January 19, 1993 in Lansing, Michigan. One item on the MAPCC's agenda was a discussion of the University of Michigan incinerators in. terms of compliance with existing State air use permits, review of a pending air use permit _

application, and discu'ssion of radioactive emissions and State and Federal regulatory authorities. Local citizens who live in the vicinity of the ,

  • Foreian Assianees On Tuesday January 19, 1993 three assignees from the Ukrainian State Committee for Nuclear and Radiation Safety arrived at NRC for training in a program -

under the Lisbon Initiative that will train a large number of assignees from Russia and Ukraine in 1993. The cssignees are dividing their time between headquarters (NRR), regional offices, reactor sites, and licensee corporate offices, studying the regulatory process and gaining experience to contribute in the development of their own regulatory program. '

Attended by the Associate Administrator, Agricultural Research Services, VSDA and staf f, and the Regional Administrator, NRC Region 1. and staff, the conference discussed repetitive violations and the apparent lack of corporate oversight provided to the 350 licensed locations in the US. 1. NFS Civil Penalty On January 19, a proposed sivil penalty of $37,500 was issued to NFS. The CP r:sulted from violations issued associated with the-fire and explosion at the high enriched scrap recovery building on September 10, 1992

($25,000), and the October 13, 1992, transfer of a solution containing high enriched uranium from a favorable to an unfavorable geometry vessel without verifying that the concentration was safe ($12,500).

2. Brunswick Civil Penalty On January 21, 1993, a proposed civil penalty of $50,000 was issued to Brunswick as a result of a failure to properly control radioactive material during an incident at Brunswick on September 22, 1992. . Region 111 Items of Interest Week Ending January 22, 1993

1. Indianapolis Power and Light Comoany On January 21, 1993, an Enforcement Conference was conducted in Region Ill with representatives of Indianapolis Power and Light Company and members of the Region 111 staff. The purpose of the conference was to discuss the findings of an inspection conducted _ October 23, 1992.

Three apparent violations and one concern were identified. The inspection findings indicated a lack of radiation safety training for "

several persons in supervisory >ositions who are involved in licensed activities. This resulted in tie unauthorized removal of fixed gauging devices containing licensed material. The licensee did not contest any =

of the apparent violations or the concern during the conference. The results of-the conference are under review and a final determination on enforcement action is pending.

2. Dresden Nuclear Station On January 22, 1993, the Regional Administrator and members of the Region 111 staff conducted an Enforcement Conference with the Vice President, Nuclear Oversight and Regulatory Services; the Dresden Site Vice President; and other members of the licensee staff, The subject of the Enforcement Conference was the use of inaccurate information submitted in support of the Technical "pecification changes to the pressure-temperature curves for Dresden Unit 2.
3. Qparability Conference Region III conducted a conference January 21 and 22, 1993, in Rosemont, Illinois -for nuclear utility representatives to discuss issues related to Generic Letter 91-18, " Resolution of Degraded and Nonconforming Operability Determinations." The meeting is also sponsored by the Midwest Nuclear Engineering Managers Forum.
4. Michioan Air Pollution Control Ccmmission'Meetina On January 19, 1993, representatives from Region 111 and the Office of-Nuclear Materials Safety and Safeguards were in Lansing Michigan attending a meeting of the Michigan Air Pollution Contro1' Commission (MAPCC). Region V Items of Interest Week Ending January 22, 1993

1. San Onofre Nuclear Generatina Station During the weeks of January 18 and 25,1993, Region V is conducting an initial operator examination at the San On.fre 2/3 Nuclear Generating Station. A total of 17 candidates are being tested by three examiners.

The licensee had uriginally scheduled 22 candidates, but after further screening selected the 17 now being tested.

2. Palo Verde Nuclear Generatina Station On January 19 and 20, 1993, the training managers for reactor licensees in Regions IV and V met at the Palo Verde Nuclear Generating Station to discuss operator training and requalification issues. This biannual meeting of the "WESTRAIN" group was attended on January 20, 1993, by NRC Mana;ement from Headquarters as well as Supervisors from Regions IV and V. Issues discussed included; Implementation of Revision 7 of the Examiners Stindards (NUREG-1021), Effects of a Proposed Rule Change to 10 CFR 55, Scope of NRC Inspections Related to ' aining, and General Scope of future Requalification Evaluations by the NRC.
3. Portland General Electric Company Representatives of Portland General Electric Company met with Region V and NRR representatives in the Region V office on January 20, 1993 to discuss their plans for the decommissioning of the Trojan Nuclear Plant.

The utility had announced on January 4 that Trojan would not restart from the steam generator tube repair outage which had begun in early November 1992. The licensee discusscd staffing and plant lay-up plans, noting that the core will be offloaded into the spent fue'l pool by the end of January, and that they hope to receive a possession-only license (POL) by the end of April 1993.
