Dear Mr. Keppler:

This letter is the final report pursuant to 10CFR50.55(e) on the significant deficiency concerning the potential for damage to the control wire insulation of 480 volt circuit breakers manufactured by Brown Boveri Electric Company.

Mr. J. McCormick-Barger of your office was notified on April 3, 1985 by Mr. P. Martin of The Cleveland Electric Illuminating Company (CEI) that this condition was being evaluated per Deviation Analysis Report 232.

Description of Deficiency In March 1985, Brown Boveri Electric Company filed a 10CFR21 report with the Nuclear Regulatory Commission concerning a potential defect in K-1600 and K-2000 type circuit breakers. A subsequent letter in March 1985 notified CEI that the potential defect also affected K-225, K-600, K-800, K-1200, K-1600 and K-2000 type circuit breakers. Of these, PNPP was supplied K-600, K-1200, K-1600 and K-2000 type circuit breakers for use in the R23 and R24 systems. Control wire insulation for these circuit breakers may experience damage as a result of contact with the top edge of the dust shield when the circuit breaker is racked out to the fully disconnected position with the compartment door closed.

Corrective' Action The. Brown Boveri Company has provided a procedure (IB-3503).

to repair and. protect the control wire insulation of the affected circuit breakers. Protection is-provided by a modification, attaching foam insulation to the dust shield' ,

which will prevent future damage.

The Cleveland Electric Illuminating Company will take the

'following actions to correct'this deficiency:

. Brown Boveri circuit breakers of the types identified as.affected.by this' deficiency and which are used in or. designated for.use_in safety-related applications-will lue inspected f for damaged control wiring.- Any

-damage identified will be' repaired. Inspections and any repairs will be' performed and documented in accord-ance with. Perry Nuclear Power Plant project procedures.

Nonconformance Reports NDS-ll8 and NDS-117 have been'

. issued to track resolution of this deficiency-for Units 1 and 2.respectively.

. Modifications designedLto prevent control wiring damage will be performed as specified by Brown Bove i Procedure IB-8503 and in accordance with project procecares. This-modification-will be performed on all affected circuit-breakers..

. . Documentation associated with the inspections, repairs and' modifications described above will be included in the closure package associated with Deviation Analysis Report 232. This-package will be available for inspec-tion after the work is complete.

The. inspections, repairs and modifications associated with this deficiency will be performed on a schedule which supports plant testing.
