SUllJECT: lladdam Neck inspection 92-21
This letter transmits the report of the resident safety inspection conducted by Messrs. W, --
Itaymond and P. Habighorst of this office during the period November 15,1992, to
.
December 12, 1992, at the Haddam Neck Plant, lladdam Neck, Connecticut The report
covers activities authorired by NRC 1.icense No. DPR-61. This inspection was focused on
issues important to public health and safety. Prelimiriary findings were discussed with Mr. J.
Stetz of your staff at the conclusion of the inspection.
-During this report period, we found lladdam Neck was operated in a safe and conservative
manner,- Our inspection validated the adequacy of procedures to mitigate a station blackout
(Silo) event. In particular, we verified risk-significant manual operator actions could be -
performed within the times required. Also, we determined actions by plant staff to address
human engineering deficiencies will enhance the ability of the plant to cope _with the Silo
. event. Finally, we found the auxiliary operator training program was a notable strength and
the plant operators performed their__ emergency duties well.
During this inspection, we identified a failure to_ perform a required surveillance on the low
pressure safety injection system core deluge valve position Since the core deluge valves are'
'
routinely _ verified to be in the correct position as part of daily panel checks, the failure to -
document this fact had minimal safety significance. Also, since you initiated adequate
corrective actions to prevent recurrence, we exercised enforcement discretion for. this case in -
accordance with Section VII,B of the Enforcement-Policy,
