Dear Mr. Giambusso:
'JJ MONTICELLO NUCLEAR GENERATING PLANT Docket No. 50-263 License No. DPR-22 RHR Service Water Pump Seal Problem A condition occurred at the Monticello Nuclear Generating. Plant recently which we are reporting to your office in accordance with the provisions of Section 6.6.C.1 of Appendix A, Technical Specifications, of the Provisional Operating License DPR-22.
The Region III Compliance office has been notified of the event.
On December 15, 1972, while operating #11 RHR Service Water pump for shutdown cooling, smoke was observed coming from the pump.
The pump was removed from service.
An investigation of the problem revealed that the upper gland seal on the pump had l
overheated due to a lack of sealing water flow which was apparently caused by over-tightening of the upper gland seal.
The upper gland seal packing for #11 RHR Service Water pump was replaced and the pump t
was returned to service on December 19, 1972. A memo has been issued to remind l
Operations and Maintenance personnel of procedures for monitoring seal water flow l
and the manufacturers instructions for packing gland adjustments.
A Significant Operating Event Report is available at the site for the Regulatory Operations inspectors.
