IPR-22 Violation of Primary Containnent A condi tion occurred at the f.bnticello tJuclear Generatine Plant which we are reporting to your of fice in accordance wi th Section 6.6.4.3 of the Technical Specifications of Provisional Operating License IFR-22.
On March 3,1973, the f.bn ticello plant was shutdown to begin a schedsled refuel-ing outage.
During the cooldown of the reactor, a torug manway was opened at a line when the reactor temperature was approximately 240 f.
This action was in violation of the speci fication which reg;i res primary containnent integri ty above a reactor temperature of 212 f.
When the error was discovered, the manway was innediatel y replaced.
It is estimated that the manway was open for about eight minutes.
The premature opening of the manway resulted when a maintenance crew was dispatched to per form the job wi thout receiving approval f rom the duty Shi f t Supervisor.
To prevent future occurrences of this nature, the torus manway covers and other primary containnent hatches will be clearly marked wi th a warning that the primary containment cannot be opened wi thou t the wri t ten permission of the Shi f t Supervisor.
The markings will be applied prior to the startup r
+t following the refuel-ing outage.
