Maintenance

During this inspection, control rod refurbishment continued.

CRDOAs 42

and 17 were completed and installed in the core, CRDOAs 39 and 16 were

removed from the core and began to be refurbished.

CRDOAs 11, 12, and 13

were in the process of being refurbished.

CRDOA 17 failed the " moment"

specification of its first in-core back EMF test.

Several subsequent

testsrevealedperformancesometimesjustpassingandsometimesjust

failing.

The cause of this performance had not been identified during

this inspection.

While inspecting some of the work procedures, the NRC inspector observed

the following:

The Authorization Sheet to Rework Task 1 of FHPWP-100-11 had not been

filled out or signed by the shif t manager and health physics prior to

starting work.

Step 11.0 of that Rework Task also was not signed by

health physics.

Task 42 of FHPWP-100-39 was completed without the health physics

notification signature required in Step 42.0.

Task'15 of FHPWP-100-39'was signed out at 5:00 a.m. on April 17,

1405, and was not signed in.by the end of the shift as required by

that procedure.

,

This!is a violation.

Independent Inspection Effort

'

'The NRC inspector found a marked " Fire Door" open.

The door to the truck

bay from level 5 of the reactor building was propped open by an air hose.

No one was in the area. When notified, operators in the control room

promptly directed the hose removed and the door shut. Other fire doors

.that were propped open were tagged with an " auxiliary system status card"

in accordance with Administrative Procedure P-2, " Equipment Clearances and

Operations Deviations."

This was considered an isolated' incident.

No violations or deviations

.were noted.

,

4.

Exit Interview.

An exit. interview was held April 19, 1985, with Mr. J. W. Gahm, Manager,

Nuclear Froduction, and other PSC personnel as denoted in paragraph 1 of

this report.' The NRC senior resident inspector also attended this

meeting.

At the meeting, the scope of the inspection and findings were
