SUBJECT:
NRC INSPECTION REPORT 50-285/92-32 (NOTICE OF VIOLATION)
1his refers to the inspection conducted by Mr. A. D. Gaines of this office
December 14-18, 1992.
The inspection included a review of activities
'
authorized for your Fort Calhoun Generating Station. At the conclusion of the
inspection, the findings were discussed with you and those members of your
staff identified in the enclosed inspection report.
Areas examined during the inspection are identified in the report. The
inspection consisted of selective examination of procedures and representative
records, interviews with personnel, and observation of activities in progress.
The inspection findings are documented in the enclosed inspect 4 )n report.
We also reviewed the circumstances surrounding Radiological Occurrence
Report 92-300 and your subsequent Root Cause Analysis Report concerning the
uptake of transuranics by one individual.
We commend you for your willingness
to confront the issues of transuranics and their implications for your plant.

The thoroughness of your review of this incident was notable.
The results of
our review are discussed in Section 7 of the enclosed report.
Your Root Cause
Analysis Report identified seven violations. Normally, in matters such as
this, we would exercise discretion in accordance with the NRC Enforcement
Policy and not cite these seven violations because they were self identified.
Even though we do not believe this matter to be indicative of a programmatic
breakdown, we are concerned with the large number of barriers that broke down
and led to the internal exposure. Also, one of violations identified was a
repeat of a violation i' itified in NRC Inspection Report 50-285/92-07.
Therefore, based on the results of this inspection, a Notice of Violation
(Notice) is issued herewith.
You are required to respond to this letter and should follow the instructions
.
specified in the enclosed Notice when preparing your response.
In your
respon_se, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.
After reviewing your response to this
Notice, including your proposed corrective actions and the' results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
