SUBJECT: NRC INSPECTION REPORT N05, 50-445/92-49; 50-446/92-49

This refers to the inspection conducted by Mr. P. M. Madden of the Office of

Nuclear Reactor Regulation, and the inspectors identified in the attached

report, on November 16-20, 1992.

The inspection included a review of

activities authorized for the Comanche Peak Steam Electric Station, Units 1

and 2, facility.

At the conclusion of the inspection, the findings were

discussed with Messrs. C. L. Terry, R. D. Walker, and those members of your

staff identified in the enclosed report.

Areas examined during the inspection included implementation of the approved

fire Protection Program for Unit 2, selective review of your design and

implementation of features to support fire protection of safe shutdown

capability, and an assessment of the ability of a fire safe shutdown to be

implemented.

Other areas examined and details are identified in the enclosed

inspection report.

Within the scope of the inspection, no violations or deviations were

identified.

However, six unresolved items were identified during this

inspection.

The six unresolved items, which are discussed in detail in the enclosed

inspection report, concern potential fire induced short circuiting and

subsequent failure of motor-operated valves required for safe shutdown; the

scope of breaker and relay preventive maintenance and testing in safe shutdown

circuitry; assumptions in the high impedance fault study required by Generic Letter 86-10; selection and application of the mally actuated fire suppression

heads; derating of emergency ligh.ing batteries in areas of elevated ambient

temperatures; and the design adequacy of a water curtain intended to satisfy

separation criteria.

The unresolved items are further discussed in the

Executive Summary of the attached report.

Further, it was found that the reactor coolant pump lube oil collection system

had neither tank level monitoring instrumentation nor administrative controls

to periodically measure and dispose of any lube oil leakage.

It is our

understanding that during the exit meeting on November 20, 1992,

Organization, connitted to have administrative controls for both units in

place prior to Unit 2 fuel load.

in a followup discussion on December 15,

1992, Mr. Walker indicated that the Unit I tanks had been sounded and drained

during the week of December 6, 1992,

if your understanding of the commitment

differs from ours, please contact me immediately.

There were also six additional items which will require inspection followup.

These are further discussed in the Executive Summary of the enclosed report.

One of these items involved numerous deficiencies found in an approved Unit 2

abnormal operating procedure.

The extent of the identified deficiencies led

to a more generic concern with your validation efforts for Unit 2 procedures.

Accordingly, a separate prelicensing inspection regarding this issue is

planned.

in accordance with 10 CfR Part 2.790 of the llRC's " Rules of Practice," a copy

of this letter and its enclosure will be placed in the NRC Public Document

Room.
