SUBJECT: COMBINED INSPECTION REPORT NOS. 50 245/92-31, 50-336/92-34,

50-423/92-30

This letter refers to the safety inspection which Mr. T. Fish, L. Briggs, and C. Sisco

conducted the week of December 7,1992, at the three Millstone Units. The enclosed report

discusses the findings and observations the inspectors made during the week's activities.

L The inspection was in the area of plant operations and was modeled after the techniques of

major team inspections, but on a smaller scale. The primary objective was to ensure that the

. facilities' operations departments conducted activities in a safe manner and in accordance with

both regulatory requirements and licensee approved procedures.

Within the scope of this inspection, no violations were obscrved. ' Operations were conducted

safely, in accordance with regulatory requirements, and in most instances in accordance with -

licensee-approved procedures, Examples where operations personnel did not adhere to .

- facility procedures were primarily in certain areas of the administrative control of operator

aids and clearances. Other observations uncovered outdated tire detector location maps in

fire panels and weak procedura.1 guidance in two surveillances. The Unit directors committed

to reviewing the inspectors' observations and correcting deficiencies where warranted.
