CRYSTAL RIVER NUCLEAR PLANT REQUALIFICATION EXAMINATION REPORT NO.
50-302/92-301
This refers to the Nuclear Regulatory Commission (NRC) administered requalifi-
cation examinations led by Richard S. Baldwin during the week of November 16,
1992.
These examinations were administered to employees of your company who
currently hold licenses to operate the Crystal River Nuclear Plant. At the
conclusion of these cxaminations, the examination questions and preliminary
findings were discussed with those members of your staff identified in the
Examination Report, Enclosure 1.
The examiners found that the Crystal River Requalification Program was satis-
factory. A Simulator Fidelity Report is provided as Enclosure 2.
A copy of
the written examination questions and answer key, as noted in Enclosure 3,
was provided to members of your training staff at the conclusion of the
examination.
We are concerned that, while your Requalification Program continues to be
satisfactory, it shows weaknesses which are a concern in both number and
severity. Of particular concern, is the inability of SR0s to properly
identify the open or closed position of the pressurizer PORV and their lack of
communication with the rest of the crew as to emergency operating procedure
'
cautions and transitions. We believe that corrective actions to address the
problems described are warranted. Accordingly, you are invited to provide a
response in writing within thirty days describing your corrective actions.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosures will be placed in the NRC Public Document Room.
