AE0D REVIEW OF 10 DINE-125 SEALED SOURCE LEAKAGE INCIDENTS (C.MICHELSON'S MEMO DTD 5/26/82)

We have no objection to preparing and issuing an information notice to supplement liotice #80-35, " Leaking and Dislodged Iodine-125 Implant Seeds."

Note that Information Notice #80-35 states that licensees (1) should

" review handling procedures for the sealed sources, including methods of sterilization, to eliminate those procedures which could physically damage I

the sealed sources," and (2) establish procedures "to assure that, following implantation, proper surveys are performed for detecting leaking or dislodged sources." We do not believe that'the issuance of a second information notice like the first one is going to eliminate the problem, but it may rekindle and heighten awareness of it. We are drafting a supplemental notice and will send it to you for review.

We believe the risk evaluation recommended in the AE0D memorandum is a good idea, and suggest another factor be considered.

If appropriate, we should also look at the " viability"_of the procedure, examining the manufacturing and quality control process to determine if they do indeed achieve an adequate degree of assurance for the structural integrity of these sources.
