LICENSEE EVENTS INVOLVING I-125 SOURCES The attached memorandum from Mr. Page is self-explanatory. There might be a general problem associated with I-125 seeds used for implants and 1-125 sources used in bone mineral analyzers.

Please examine these reports in accordance with the new event review procedure. Please provide a summary statement to me along with recom-mendations for follow-up action if appropriate.

Coordinate with Mr. Singer and Dr. Terry as appropriate.
