Dear Mr. Hall,

In accordance vith Part III, Section 12, this letter is serving as confirmation-of a telephone report of an oil spill made to Druce Hiller on 12/8/92.

1. The limitation which has been exceeded: No permit limitation has been exceeded; no visible sheen was present-on Lake Erie. The oil was discharged from the plant out fall (No. 3IB00016004),. which does not _ have 1

an oil and grease limitation. Outfall 3IB00016002 has an oil and grease limitation of _15 mg/L 30 day and 20_ mg/L daily. ' Neither of thra limits vere exceeded. This event is being reported under Part III, Section

12. A.l. - Any noncompliance which may_ endanger health or _ the environment.

2.- The extent of the exceedance: Approximately 12.5 gallons per day of of

. turbine lubricating oil vere mixed with plant service rater and discharged to Lake Erie. The concentration at the plant discharge (outfall.

3. The cause of the exceedancet Turbine lubricating oil' leaked from the oil:

Stratma P110NE 5740 ROOM S270 SUlWECT Resolution of Procedure Problems / Concerns During a recent NRC inspection, Perry received a violation for an landequate procedure. The NRC personnel performing the inspection indicated that individuals at Perry had expressed concerns about the_ adequacy of the procedure in question but failed to express those concerns to management.

A similar event involving a procedural noncompliance was documented in LER 92-022. In this instance steps were taken to work around a perceived procedure deficiency rather than resolving the procedure problem. The efforts to work around the problem resulted in a violation of the plant Technical Specifications.

PAP-0502 contains guidance for initiating changes to procedures when required. This process should be utilized when problems are encountered involving site procedures. Contact your immediate supervisor to resolve any problems identified prior to proceeding when written instructions are in question.
