NRC INSPECTION REPORT N0. 50-458/92-32 (NOTICE OF VIOLATION)
This refers to the inspection conducted by Mr. W. F. Smith, and the inspectors
identified in the attached report, of this office September 27 through
November 7, 1992. The inspection included a review of activities authorized
for your River Bend Station facility.
At the conclusion of the inspection,
the findings were discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examination of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed ' Notice of
Violation (Notice). The first violation is of concern because your licensed
operators failed to perform a containment purge in accordance with the system
operating procedure. This violation, though identified by your staff, is
being cited because corrective actions addressing the failure to follow
procedures were not implemented by the end of this inspection period, which
was approximately 6 weeks after the event. The second violation, which
involved your operator's failure to perform the required off-site power
transmission network availability verification within an hour-of placing the
Division I diesel generator in an inoperable status, is of concern because it
showed a poor practice of not entering Technical Specification action
statements for planned, short duration equipment outages. The third violation
is of concern because it was an example of maintenance procedures not
providing adequate instructions to satisfactorily complete the work. This
violation is being cited because your immediate corrective actions _did not
identify and correct all bent pushrods prior to running the engine.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.

actions you plan to prevent recurrence.
After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
Four other violations were identified in this report.
However, in view of the
minor safety significance of each violation, and the good performance-of your
staff in documenting the events and executing corrective actions for each
violation, the violations meet the criteria specified in 10 CFR Part 2,
Appendix C,Section VII.B.1, of the Enforcement Policy for noncited violations
and no enforcement action will be taken.
It is our understanding that your staff has made a commitment to adjust the
security access computer program data as necessary to deny any individual
access to the protected area when the individual's general employee training
has expired, and that the estimated completion date for full implementation
was December 1, 1992.

us.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of-
this letter and its enclosures will be placed in the NRC's Public Document
Room.
The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
