d DUKE POWER COMPANY REPLY TO A NOTICE OF VIOLATION 413,414/92-24 02 Technical Specification 6.8.1 requires in part that written proceduros be established, implomonted and ma<ntained covoring the activitios referenced in Appendix A of Ro0ulatory GJ!do 1.33, Rovision 2, February 1978. Maintenanco Proceduro MP/0/A/7650/115, Erection and Romoval of Scaffeiding, prohibits scaffolding from bein0 secured to piping loss than throo inchos in d'amotor, prohibits scaf folding from twing secured 19 piping with mirror insulation, requires that security / stacking pins bo installed in all sections, and requires that docking /plankin0 and toeboards bo secured.

Contrary to the abcve, MP/0/A/7650/11b was not followed on July 15,1992, when i

maintt,nanco personnel orocted scaffolding in the 2A and 2B Diosol Gonorator rooms in that they secuted scaffolding to 3/4 inch piping, socured scaffolding to piping with mirror insulation, failed to install security /stackin0 pins in all scatfolding sections, and fallod to securo certain sections of docking /plankin0 and tooboards.

This is a Severity LevelIV Violation.

RESPONSIE:

Reason For Violfti!QD This violation is attributed to inappropriato action for not following tho scaffolding gonoralguidelines by maintenanco personnoi rosponsiblo for oroction of scaffolds due to a lack of attention to dotail. Also, the scaffolds woro erected in an unusual and difficult location, which compounded the problem of following the procedure requisitos.

A walkdown was performed to inspect orected station scaffolding to ensuro scaffolding currently in use was crocted in compliance with scaffolding proceduro requiromonts.
