Dear Mr. Reed:

This refers to the inspection conducted by D. Hills, C. Phillips, and

M. Miller of this office, and by J. Roman of_ the Illinois Department of

Nuclear Safety, on October 14 through November 25, 1992. The inspection

,

included a review of authorized activities for your LaSalle County Station.

At the conclusion of the inspection, the findings were discussed with those

members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in _ the report. -Within

these areas, the inspection consisted of select'ive examinations of procedures

!

and representative- records, interviews with- personnel,_ and observation of

activities in progress.

During this inspection, certain of your activities appeared to be in violation

.

of NRC requirements, as specified in the anclosed Notice of Violation

j

(Notice).

These violations are of concern as they represent a-lack of

attention to detail during refueling operations, . routine operations',. and in

the preparation of procedures. This problem.was further-emphasized by the:

-

failure of operations personnel to recognize that authorizing:an out-df-

.

service was not consistent with the current plant configuration.

The-

significance of these instances ranged from two fuel assemblies being -loaded

l

in'the incorrect storage locations to reactor scrams._ We request a written

i

response within-30 days describing your actions to address the lack;of

~

attention to detail at your station. We are concerned with the effectiveness

i

of management in taking lasting corrective actions.

As.a result, we would

like to discuss this report and subsequent events at a management maeting-

i

l

which will be scheduled with you in.the near future.

Other activities were also identified to be in violation of NRC requirements,

not be subject to enforcement action because the criter.ia specified -in-Section

VII.B of the " General Statement of Policy and Procedures for NRC Enforcement-
