Items of Interest Week Ending November 13, 1992 Thermo-laa Fire Barrier Endurance Tests Members of the Plant Systems Branch witnessed Thermo-Lag 330-1 fire barrier fire endurance tests performed by Omega Point Laboratories (OPL), San Antonio, Texas, for Texas Utilities Electric Company on November 3 and 5, 1992. OPL and the licensee declared both tests successful based on satisfactory post fire inspections of the fire barriers and the cables. Temperatures taken from the conduit surfaces were indeterminate.

The first test assembly consisted of 3/4-inch, 3-inch, and 5-inch conduit specimens with lateral bends (LB0s) and radius bends. The second assembly consisted of two 3-inch conduit specimens with junction boxes. The fire barriers included a combination of vendor-recommended assembly methods and licensee-designed upgrades. Both test assemblies were exposed to 1-hour ASTM E-Il9 standard fires followed by fog nozzle hose stream tests of 5 minutes duration. Each cable's insulation resistance was checked by mega-ohmmeter before the fire exposure and immediately following the hose stream test. Following the. hose stream tests, OPL and the licensee inspected each fire barrier. Then the

laboratory disassembled each barrier and visually inspected its component pieces. OPL and the licensee determined that neither barrier was breached in any way. There were no burn-throughs, open seams, or open joints., following the ,ost fire resistance tests, OPL removed the cables from the cenduits and inspected each cable for visual fire damage. The laboratory and the licensee concluded that .the cables were not damaged during the tests.

During the fire exposure, some temperatures taken from the conduit surfaces appeared to be inconsistent with visual observations. The temperatures were irregular and higher than expected. Post fire inspection revealed that the insulation of some of the thermocouple wires located on the surfaces of the conduits were saturated with a gummy substance that appeared to be a mixture of water and Thermo-Lag material. OPL theorized that this affected the temperature measurements. Therefore, temperatures taken from the conduit surfaces were indeterminate.

The test specimens included upgrades. Therefore, the results of these two tests do not affect NRC Bulletin 92-01 or its supplement. The licensee will be conducting additional fire endurance tests during November.

Sandia Testino of Okofie_f?Rnwalon Cables During testing conducted by Sandia National Laboratories under contract with the Instrumentation and Controls Branch, the ten Okonite ethylene propylene rubber-(EPR)/Hypalon cable samples (cable samples of other materials from two other vendors were also included in the test) did not survive the LOCA test.

Prior to the LOCA tests the cable samples had been subjected to 130 megarads NOVEMBER 13, 1992. ENCLOSURE A

1 of radiation aging and thermal aging equivalent to 40 years of plant life at 69'C. The staff is currently evaluating the Sandia test data and other available qualification type test data on these Okonite cables (0konite has performed qualification tests to the requirements of IEEE-Std-323/1974). In other similar tests conducted by Sandia for flRC Research, one out of four samples aged to a thermal life equivalent to 40 year life at 56*C failed during LOCA testing. The staff will be issuing an information notice to licensees to alert them of potential safety concerns. There is no immediate safety c, tern since other available test data indicates acceptable cable perf ormance at 50*C service life; most installed, safety-related cables do not experience continuous service life conditions above 50*C; and plants have not reached 40 years of life as simulated in the Sandia tests. Planned staff actions include evaluation of the long term safety significance of this issue.

Motor-Ourat_ed Valve Inspection at LaSalle fluclear Power Plant From flovember 2 to 6,1992, the Mechanical Engineering Branch participated in an engoing Region 111 inspection of the program being developed at the LaSalle nuclear power plant in response to Generic Letter (GL) 89-10, " Safety-Related Motor-Operated Valve Testing and Surveillance," and other motor-operated valve (MOV) activities.

During a meeting with the NRC staff in April 1992, Commonwealth Edison Company (CECO) committed to evaluate data from MOV tests performed at its facilities in response to GL 89-10. In accordance with 10 CFR Part 21, Liberty Technologies notified the flRC staff and nuclear power plant licensees in October 1992 of increased inaccuracy of its V0TES diagnostic equipment in certain instances when used to measure the thrust delivered by motor actuators in opening and closing valves. CEC 0 has completed a preliminary evaluation of the MOV test data including, where applicable, an initial effort at addressing the effect of the increased inaccuracy of the V0TES equipment. The CECO preliminary assessment indicates that the 0.3 valve factor previously assumed by CECO in its MOV sizing and switch setting calculations may underestimate the amount of thrust needed to overcome differential pressure in opening or closing many valves. Further, the preliminary assessment reveals that the -

reduct!cn in thrust output of the motor actuator caused by load sensitive behavior (rate of loading) can be significant for scme MOVs.

The licensee has conducted tosts at laSalle of several motor-operated gate and globe valves under differential pressure and flow conditions in response to GL 89-10. Using new guidelines provided by Liberty Technologies (which have been incorporated into new VOTES software), the licensee determined that, in some cases, the VOTES equipment underestimated the amount of thrust delivered by the motor actuators for tested MOVs by up to 40 percent. The licensee is evaluating several potentis) operability concerns identified using its acceptance criteria for the tested MOVs.

The inspectors identified weaknesses in the licensee's acceptance criteria for MOV test results. For example, the acceptance criteria did not include consideration of diagnostic equipment inaccuracy in evaluating certain aspects of the capability of the tested MOV. The acceptance criteria also did not {

g 1 consider torque requirements or load sensitive behavior to ensure motor actuator apability under degraded voltage conditions.

For the specific MOVs tested at LaSalle, the licensee had set the torque switches sufficiently high to allow their motor actuators to deliver adequate i thrust to open and close the valves during the tests and, as extrapolated, ,

under design basis conditions, Nevertheless, the MOV test data indicated that r most of the tested gate valves required more thrust to overcome the differential pressure and flow effects than predicted by the licensee's calculations. In its-analysis of the test results (including the underestimation of thrust by the VOTES equipment), the licensee determined the' t valve factors for the gate valves to range up to 0.9 using the outer valve  ;

disk diameter. Many of the tested MOVs showed load sensitive behavior that ,

reduced the thrust delivered by their motor actuators by as much as 23 percent-under dynamic conditions as compared to static conditions.

Region Ill will document these and other findings in an inspection report when the inspection is completed later in November 1992.

FERMI-2 On November 4,1992, the licensee commenced a reactor startup following the.

completion of the third Fermi-2 refueling outage. The outage,: originally .

scheduled for 50 days, was completed in 54 days. The reactor went critical at 11:57 PM. The project manager and backup project manager were on site and in the control room, augmenting the resident inspector coverage ~providing '

backshift support. The licensee is currently conducting a lengthy three-week' startup test program following Power Uprate modifications. Other major work completed during the outage includes reactor pressure vessel (RPV) in service.

Dresden. Units 2 and 3 On November 10,-1992, Commonwealth Edison Company-(Ceco) announced the appointment.of Michael J. Lyster (presently Vice President - Perry Nuclear Power Plant), as Site Vice President - Dresden Nuclear Station. This is the first of the newly created Site Vice President positions to be filled. CECO has' stated that an announcement regarding the other Site Vice President -

positions is expected soon.

On November 4, 1992, Transportation Branch staff participated in a meeting with staff from the Departments of Transportation, State, and Energy. The meeting was held to discuss requirements for sea-going vessels for transport of irradiated nuclear fuel. A draft code was developed by the International Maritime Organization (IMO), which would require specific design charac- ,

teristics for sea-going vessels carrying irradiated fuel. The consensus of the group was that reasonable requirements are appropriate for sea-going vessels. The U.S. Coast Guard will be representing the United States in a subsequent meeting with IMO.

Nuclear Fuel Services. Inc. (NFS) - Downblendina of High-Enriched Uranium (HEU)

On November 5, 1992, staff from the Office of Nuclear Material Safety and Safeguards and Region 11 met with NFS representatives. The primary focus of the meeting was to discuss NFS' October 15, 1992, submittal regarding a process for downblending HEU to low-enriched uranium for delivery to light-water reactor fuel fabricators. The submittal includes an update to the safety demonstration section of the license describing the proposed process and a supporting safety analysis consisting of an operational hazards analysis u,ing a failure modes and effects analysis technique and a probabilistic risk-assessment.

Quality Management proaram a3d Misadministrations Rule - Public Meetina ,

On November 9,1992, staff from the Office of Nuclear Material Safety and Safeguards conducted a public meeting with representatives of various professional organizations in Chantilly, Virginia. Participants included 28 professional societies or associations involved in the medical- application of by-product material. The purpose of the meeting was to ensure a mutual understanding of the " Quality Management Program and Misadministrations" Rule (the Rule) and discuss effective implementation. Additionally, the American College of Nuclear Physicians (ACNP) was to present its Practice Audit Program and discuss how their voluntary audit program might fit into the implementation of the Rule required by the Nuclear Regulatory. Commission.

Other professional organizations that may be affected by the Rule were encouraged to comment regarding any self-auditing or voluntary quality assurance programs and how such programs might compare to the Rule.

4 The staff and participants also discussed the Rule language, regulatory guide,

, inspection guidance,-and proposed enforcement policy as well as ACNP's audit program. Participants commented on the meeting's-success and expressed interest in future' meetings to discuss NRC recognition of the ACNP Practice Audit Program in nuclear medicine. Additionally, participants showed strong interest in participating in future public meetings as part of the development of a management plan for the medical use program.

Laboratory astlJ During November 2-5, 1992, the quality assurance (QA) staff of the Division of High-level Waste Management (HLWM) and the Center for Nuclear Waste Regulatory Analyses (CNWRA) observed a limited QA audit of LANL performed in Los Alamos, New Mexico, by the Yucca Mountain Quality Assurance Division (YMQAD) of the Department of Energy Office of Civilian Radioactive Waste Management (0CRWM).

This audit, YMP-93-02, evaluated the LANL QA program to determine whether it meets the OCRWM requirements and commitments. Six QA programmatic elements and four technical areas were evaluated by the YMQAD audit team.

The HLWM and CNWRA observers determined that the audit was effective and agreed with the YMQAD aud t team that inplementation of the LANL QA program was adequate in all of the QA programmatic elements that were audited. As a result of the audit, two preliminary Corrective Action Requests (CARS) were issued bv the YMQAD audit team, both in the area of software QA. The adverse conditions identified in the CARS are not significant in terms of the overall LANL QA program.

Quality Assurance iQAJ ReviSw of Site Activitiej On November 6, 1992, Division of High-level Waste Management (HLWM) On-Site Licensing Representatives reviewed and evaluated Yucca Mountain Site Characterization Project activities at the Nevada Test Site pertaining to the drilling, coring and processing of core. This review was conducted at the Sample Management facility, at the LM300 drilling rig station UE-25-UZl6 and at the CME 850 drilling rig station USW-UZ-N58. The UZl6 borehole is at a depth of 980 feet and planned for a depth of approximately 1660 feet. The UZ-N58 borehole is at a depth of 10.14 feet and planned for a depth of approximately 120 feet, lhe purpose of this review was to determine the extent to which activities are being performed in compliance with approved Study Plans, Job Packages, Test Planning Packages, Verification Plans, and quality-related procedures and instructions. This review involved interactions with Reynold Electric and Engineering Company (REECo) personnel responsible for drilling operations; Raytheon Nuclear Services personnel responsible for the verification and monitoring of drilling and coring depth measurement; Sample Management f acility personnel responsible for the identification, filming, packaging, and processing of core; and Yucca Mountain Quality Assurance Division personnel responsible for the overall monitoring of site activities.

Overall, it was determined that the personnel interviewed were knowledgeable of procedural requirements as they pertained to their individual job furctions and that the activities observed were being conducted in accordance with the appropriate QA program quality-related procedures. In addition, the qualification and training records of the personnel interviewed were reviewed and it was determined that they were qualified, and trained in the appropriate procedures.

h Construction Strategy sponsored by the Nuclear Waste Technical Review Board (NW10B), Panel on Structural Geology and Geoengineering. The agenda included pres!ntations and discussions on the technical merits, costs, and schedules i for atrategies for earlier and less costly underground construction and testing in the ESF. Presentations were made by NWTRB members and consultants, the Department of Energy (DOE), contractors to DOC, the U.S. Geological r Survey, Lawrence Livermore, Los Alamos, and Sandia National Laboratories. The discussions included the benefits and drawbacks of different excavation methods, exploration and testing strategies, management and contracting methods, and the concept of an off-repository block testing facility at Busted - ,

Butte. Nuclear Regulatory Commission staff, although not part of the agenda, i did make comments to ensure that the role of the NRC in the regulation of the  !

disposal of high-level waste is not misunderstood, in addition to those mentioned above, the panel meeting was attended by representatives of the State of Nevada and affected counties, and by members of tne news media.

Transcripts of the workshop were made, and the results of the discussions are ,

to be presented in a topical report by the NWTRB and also in the report by the r board to Congress.

NRC Staff Visit to Canada's Whiteshell Nuclear Research Laboratories and Underaround Research Laboratory On November 3-4, 1992, Division of High-level Waste Management (HLWM) staff visited the Atomic Energy of Canada Limited (AECL) Whiteshell Nuclear Research Laboratories and Underground Research Laboratory (URL) and conducted a technical exchange on repository engineered barrier system (EBS) and geotechnical engineering issues of interest. Specifically, AECL staff at .

Whiteshell made presentations on their ongoing research activities related to spent fuel oxidation and dissolution and provided tours of the laboratories and hot cell facilities where the studies are performed. In turn, HLWM staff gave an overview of Nuclear Regulatory Commission interest in spent fuel research, reflective of the dominant role spent fuel will have in the U.S.

high-level waste management program. It is worth noting that, notwithstanding significant differencet in CANDU reactor and fuel assembly design in comparison with U.S. designs, the dominant spent fuel technical issues (i.e.

spent fuel oxidation and dissolution) are the same in our respective programs.

AECL staff at the URL gave an overview of their activities related to thermal, mechanical and thermo-mechanical modeling for the disposal vault engineering.

Presentations included discussions of the experiments at the URL such as the excavation disturbed zone studies, buffer /cor,tainer experiment and mine-by experiment. HLWM staff made a presentation on NRC geotechnical engineering activities, including proactive, reactive and research work that is currently going on in the area of repository design, construction and operation. The presentations were followed by a tour and description of ongoing research at both the 240 m and 420 m levels of the URL. Although the rock type at the URL is massive granite that differs significantly from the fractured tuff at the Yucca Mountain site, the experience gained in the areas of instrumentation-development for the-in-situ testing and the model development and validation at the URL are very applicable to the U.S. program. It should be noted -

that the work in progress at the URL is to demonstrate the technology and the concept of deep geologic disposal and not to demonstrate the suitability of that site for the ultimate disposal of the Canadian high-level waste.

Comnletion of Pathfinder Generatino Plant Dismantlement On flovember 10, 1992, the Nuclear Regulatory Commission staff authorized, by license amendment, the unrestricted release of the Reactor Building,- Fuel Handling Building, and Waste Storage Building, at Northern States Power's '

Pathfinder Generating Plant and the demolition of the Reactor Building.

This action-completes dismantlement efforts at Pathfinder, which have been conducted in accordance with the " Environmental Assessment of Proposed final Decommissioning of the Fuel Handling Building and Reactor Building at the PathfinderGeneratipgPlant,"datedJune1990. The dismantlement activities f

generated 34,450 ft of low-level waste and resulted in occupational exposures totalling 3.984 man-rem. The total cost, including projected costs to demolish the Reactor Building, will be $13 million.

Transfer of AMAX Site to the DenJrtment of Energy On November 3, 1992, Nuclear Regulatory Commission staff received a letter from the Department of Energy (DOE) regarding their schedule for transferring title of the AMAX site in Wood County, West Virginia, to DOE. DOE indicated that they would accept transfer of the site prior to Ma ch 31, 1993, and stated that they had reached agreement with AMAX on the amount of a lump-sum .-

payment to cover _ future _ costs of site monitoring and maintenance.

- Office of Nuclear Regulatory Research Items of Interest Week Ending November 13, 1992 Fission Product Re_tintion by_W_a_ter Overlyian Core Debris 5

Draf t NUREG/CR-5901, "A Simplified Model of Aerosol Scrubbing by a Water Pool Overlying Core Debris Interacting.With Concrete," was issued for comment in October 1992. This report documents work done to provide technical support for the NRC Staff's effort to develop updated accident source terms for light-water nuclear power plants (NUREG-1465). The revised source term report outlines the rates at which various groups of fission products are expected to emerge from a degrading core into the reactor containment atmosphere.

Simultaneously with the release from the core, the action of installed safety systems, along with certain natural fission-product removal processes, tends to reduce the airborne radioactive inventory in the containment atmosphere.

This draft report (NUREG/CR-5901) analyzes the natural process by which aerosol-laden bubbles are scrubbed by a pool of water overlying the core debris. Comments by interested members of the public are being solicited by a Federal Register notice issued on November 5, 1992 (p. 52798).

Wo_rlshop on Organizatjonal Fagtors Research On November 12, 1992, DSR/HFB conducted a workshop on organizational factors research to inform senior management of research products to date and to seek recommendations for the future of the research. Participation included J. Sniezek, T. Murley, T. Martin, S. Ebneter, J. Milhoan, D. Ross, L. Spessard, J. Carroll, W. Lindblad, D. Ward, and others. The participants felt that work being brought to an orderly conclusion could be of assistance in inspections and for Diagnostic Evaluation Teams, and progress had been made in cons;dering organizational factors in PRAs. NRR and AE0D will identify any needs for continued research in this area in later years. The current work will be completed in FY 1993.

1111-2 Vessel Investigation Project Program Review Group and Management Board Meeting The TMI-2 Vessel Investigation Project (VIP) Program Review Group (PRG) and Management Board (MB) met on November 12-13, 1992, in Bethesda, MD. The main subject discussed at the PRG meeting was results of preliminary margin-to-failure scoping calculations of the TMI-2 reactor pressure vessel. Instrument tube penetration failures and local and global creep rupture failure modes were considered. France and Japan also presented results of their nozzle and guide tube examinations and TMI companion sample examinations, respectively.

Because of the continued hot cell shutdown at INEL, the PRG agreed not to pursue any further examinations of the 8 nozzles at INEL. Instead, additional analyses will be performed to expand on and better understand the results of the preliminary scoping calculations. The next and final PRG and MB meetings will be held on May 17-19, 1993, at Argonne National Laboratory, to discuss comments on the project's final report. Also, a final event to present the results of the TMI VIP is scheduled for October 20-22, 1993 in the Chicago or Washington, DC area.

A joint NRC/INP0/ industry task group is reviewing therlessons. learned from Turkey Point?s encounter with Hurricane Andrew. Following a~ week at Turkey Point gathering information, the 8-member team collected in Bethesda the week ,

of November 9 to begin the task of analyzing the data and preparing the first -

draft of the team ryort. The report is scheduled to be issued by December 22, 1992.

Incident Response Branch (IRB)

On November 9, the AE0D Director and IRB staff provided a tour of the Operations Center and an extended briefing on emergency-response to officials from Guangdong Province in the People's Republic of China. The provincial officials were particularly interested because a plant is coming.on-line in their province in 1993.

On November 10, IRB staff provided a tour and briefing.on the Operations-Center to utility Emergency Response Coordinators (ERC) in Regionill. The NRC ERC from Region II attended. Members of NRR briefed the group.

The Library Bulletin is a listing of recent additions to the Library's collections that includes monographs, technical reports, conference proceedings, International Atomic Energy Agency publications, and documents in subject areas such as nuclear science, engineering, waste management, health physics, and other disciplines associated with the mission of the flRC. All items listed in the library _ Bulletin are available for loan to the liRC staff.

October 30 - November 3, 1992, in Atlanta, Georgia. The theme of the conference was " Thriving in a Changing World," Conference topics included sexual harassment, ethics, depressicn and anxiety disorders in the workplace, critical incident stress debriefing, addiction treatment outcome measures, and compulsive spending and gambling.

The NRC participated in the Hispanic Alliance for Career Enhancement Annual Conference in Chicago, Illinois on November 13-14, 1992. The Conference attracted a variety of applicants from technical, administrative, and clerical ,

occupations. NRC interviewed a total of 18 engineers and scientists. _These applications will be referred to several offices within the agency for follow-up.

Office of Consolidation items of Interest Week Ending November 13, 1992 Second Buildina Status During the week, work continued on the waterproofing and finishing of the roof and window installation. Grading and placement of underground utilities continued behind OWFN and TWFN. An 8" water line connection to the street was c ompl e ted . Plumbing rough-ins for toilets and drinking fountains completed on floors 2-10. Drywall enclosure of elevator cores completed on floors 2-10.

On November 9, we notified the developer that the Commission had no objection to the selection of David Kraisler's "Architype Circular" as the sculpture for -

the site.

Also on November 9, the Director presented a statement of work to the SCRB for competitive procurement of the ergonomic seating for work stations in the second building.

On November 10, Office staff participated in various meetings including a presentation by the space planning sub-contractor DBI of design concepts for the Training Center and Fitness Center, a presentation by HVAC sub-contractor LDE of a cold air diffusion system for the second building, a weekly status meeting on the TRW design project for the Operations Center, and a weekly TWFN project status meeting with GSA and Lerner Enterprises.

On November 13, the Director attended a meeting of the Partnership for Regional Excellence sponsored by the Metropolitan Washington Council of Governments. The Partnership is examining the fundamental issues of growth, transportation, environment and governance for the metropolitan region encompassing the District of Columbia and portions of Maryland and Virginia, i

Internat *onal LLW Conferenct Stephen Salomon, State Prograna, attended the International Low-Level Waste (LLW) Conference sponsored by the Electric Power Research Institute on November 9-12, 1992 in Baltimore, Maryland. This technical conference focused on exchanging information new technical developments and interna'*onal approaches to the processing of dry and liquid wastes generated by nuclear power plants. Mixed wastes and decontamination technology were also discussed.

Subcommittee on the Ecot;mics of Natural Resources and the Environment Maria E. Lopez-Otin, Federal liaison, attended the first meeting of the Committee on Earth and Environmental Sciences, Subcommittee on the Economics of Natural Resources and the Environment (ERNRE), on November 10, 1992. The purpose of ERNRE is to improve the knowledge base in economics'needed for-understanding the sources and tha effects of environmental and resource changes, and the policies and practices adopted as responses to these changes.

In this regard, the ERNRE expects to devote its first year working in areas of-economic research which may' be of interest to the NRC (e.g. valuation of environmental / natural resources, including water resources, integrating the costs of mitigation, forecasting natural resources economics). 'These and other topics will be discussed at future meetings and a few will be selected for ERNRE development.

1. State University of New York. Buffalo Materials Research Center The lunch relief operator of the Buffalo Materials' Research Center two '

megawatt non-power- reactor.-was discovered sleeping by member of the facility staff at about 12:15 p.m. on November 5, 1992. The operator was awakened, but subsequent questioning of the regular operator indicates the relief operator may have again been inattentive when'the-regular operator returned from lunch about 15 minutes later. The licensee removed the operator from licensed activities pending completion of an investigation and has indicated they will seek NRC concurrence before returning the individual to duty. Region I will conduct an inspection of- the incident after the licensee completes its.

1. Commissioner Curtiss visit to GE qJ On November 12, 1992, Commissioner Curtiss accompanied by.the Director, Division-of Radiation-Safety and Safeguards, and other staff visited the General Electric Fuel Fabrication Plant in Wilmington, North Carolina.

In addition to attending overall facilities and program status presentations, Commissioner Curtiss toured the fuel plant,. including-areas where fuel element mechanical components are fabricated.

2. Enterav Operations. Inc.

On November 13, the Regional Administrator, accompanied by the Deputy Director, Division of Radiation Safety and Safeguards, was at the Grand 1

Gulf site to present the latest MLP results to representatives of Entergy Operations, Inc., and to nold a public meeting with-local officials attending the SALP presentation.

3. Carolina Power and Licht Company .

J On November 11, CP&L President William Cavanaugh III announced the following management changes at the Brunswick Nuclear Plant:

. R. B. Richey, currently vice president of the Brunswick Nuclear Project,-will begin in early December a special assignment for CP&L with the United States Council for Energy Awareness (USCEA).

Based in Washington, D.C., USCEA is a_ private international trade and communications association of the nuclear power industry.

Sherwood H. Smith, Jr. currently serves as vice chairman of USCEA and will be the organization?s. chairman during 1993.- 1994.

. Until Richey's replacement at the plant is named in the near ~

future, Dick Morgan will serve as the interim manager for..the-overall Brunswick Nuclear Project. Morgan is currently serving as the interim plant manager for Unit 1. He was named to that position last month and will_ continue to hold that position as

, well during this period.

1. Clinton: Nuclear Power Station The Regional Administrator, and Director,. Division of Reactor Projects, and members of the Region Ill staff met eith the Senior Vice-President-of Illinois Power Company and members of his staff, on November 10, 1992, at the Clinton Station. The purpose of the routine meeting was_to discuss the station's performance.
2. Duane Arnold Enerav Center On November 12, 1992, the Regional Administrator, the Director, Division -

of Reactor Projects and other staffers from Region III and NRR, met publicly with the Chairman of the Board and Chief Executive Officer, Iowa Electric Light and Power Company, and other senior licensee management representatives and staff, at the Duane Arnold Energy Center to present the initial SALP 10 report. The meeting was attended by licensee staff, state representatives and the media. The licensee 1 indicated it was in agreement with the assessment. _

3. Kaiser Aluminum An Enforcement Conference was conducted with this Company on November 10, 1992 to address the Company's failure to maintain control over a generally licensed gauge. A building in which the gauge was. located, was transferred to another company, and the gauge was subsequently sent ',

to a scrap yard, where it was identified as containing radioactive-material. Kaiser Aluminum developed an extensive program to assure proper control over the 60 additional gauges it possess at multiple locations. Escalated enforcement is being considered. While this was an open enforcement conference, no members of the public attended. _

4. Radioactive-Material in Public Domain  ;

On November 9,1992, an anonymous caller stated that he had measured contamination and direct radiation levels in an industrial area' in Hammond, Indiana. Two radiation specialists were sent _to the site.  ;

They found one area on the ground, near two trailers which were parked i in a lot, that read 5 mR/hr. They determined the material to be cesium a 137. They removed the material completely by scooping up a small amount

of soil. They entered the trailers with local authorities, and found-several empty barrels and a scale, but no additional radioactive ,

ma terial s . The region is continuing to follow leads regarding possible use of radioactive materials at the site.

5. Dresden Oversicht Team The Dresden Oversight Team, comprised of the Deputy director, DRS, and:

other members of headquarters and Region III staff, conducted a 3 day -

1 licensee was making good-progress but much work remains to be_done. As.

a result of consultant, _INPO, and NRC input, the licensee acknowledges the need for_.significant change and is focusing improvement efforts on organizational effectiveness, management effectiveness, business .

6. Commonwulth Edison Corrnany (Ceco)

CECO announced that Site Vice Presidents (Site VP) have been selected for five of its six sites. They and Mike Wallace,' Chief Nuclear-Operating Officer designate, will be removed from most line organization- .

responsibilities beginning November 16 for transition activities which include determining organizational details. These individuals will assume their new responsibilities December 17, 1992. The five Site VPs selected are:

Braidwood - Sigval Berg, Jr. formerly Technical Superintendent, Dresden Byron - Kenneth Graesser, formerly General Manager, BWRs Zion - Richard Tuetken, formerly General Manager for Zion Station Dresden - Michael Lyster, formerly Vice President, Nuclear for Perry Station (Cleveland Electric _ Illuminating Company)

Ouad Cities Richard Pleniewicz, formerly Plant Manger, Byron The LaSalle Site VP is anticipated to be selected from outside Ceco and appointed after the Christmas holidays. Gerry Diederich, LaSalle Plant Manager will be Acting Vice President until the permanent Site VP is on board.

7. Ceco Rate Hearina The Illinois Commerce Commission (ICC) staff recommended to the ICC that the full cost of both Byron units and Braidwood Unit-1 be considered used and useful for the rate decision expected to be made in January.

If the ICC accepts this recommendation in making its rate decision and: -

the State Supreme Court upholds that decision, CECO's financial pressures could be reduced. Previously, the_ State Supreme Court had ruled that only one Byron unit and neither Braidwood unit were used and--

useful. This resulted in rollback-of a rate increase which included--the construction cost of the three units.

1. Commissioner de Planaue toured New Mexico Uranium Recovery Sited ,

Commissioner de Planque-. toured uranium recovery facility sites located in the vicinity of Grants, NM, November 9, 1992. The Commissioner was accompanied by the Regional Administrator, Region IV, the Commissioner's Technical Assistant, and by the Director, and a Project Manger, .from the Vranium Recovery Field Of fice. Sites toured included the Homestake Milan, Quivira Ambrosia Lake, and the ARCO Bluewater uranium mill sites.

In addition, the Commissioner briefly visited the Ambrosia Lake Title I i reclamation site, which is in the initial phase of reclamation.

2. Seouoyah Fuels Corporation k

On November 6, 1992, the U.S. Environmental Protection Agency (EPA) j issued a " draft" Administrative Order on Consent (Order) to Sequoyah i Fuels Corporation (SFC) pursuant to the provisions of Section 3008(h) of -!

the Solid Waste Disposal Act as amended by the Resource Conservation and  ?

P,ecovery Act (RCRA) and the Hazardous Solid Waste Act (HSWA). The Order requires that SFC take actions to assess the extent of site contamination by hazardous materials covered by HSWA, to mitigate further spread of the contamination, and begin remediation of the site.  ;

in a letter transmitting the Order, EPA noted its desire to complete negotiations regarding the Order with SFC within a-period of 60 days.

The purpose of the negotiations is to develop a schedule and plan for interim measures to be taken at the site to minimize further spread of contamination while long-term remedies are pursued, and for SFC to develop and obtain approval for a RCRA Facility Investigation. EPA also-informed SFC that if such negotiations were not completed within the 60- -

day period, that EPA would seek other options to address- the contamination present at'the Sequoyah facility, c Among other tasks, the Order requires that SFC (1) develop and. implement an Interim Measures Workplan, (2) perform a RCRA Facility Investigation,

(3) perform a Corrective Measures Study, and (4) perform a Corrective l Measure implementation implementing the corrective measures selected by EPA for the facility. The Order also requires that SFC establish an escrow account in the amount of $5,000,000 within 10 days of the

l effective date of the Order to ensure that required corrective actions are successfully completed, EPA does not plan to issue a press release regarding the subject Order at this time. 'However, EPA will make the final Corrective Measure-Study report and a summary of EPA's proposed corrective measures for the site

! available for public comment. After consideration of public comments,

!. EPA may require that SFC modify its proposed corrective measures.

Emergency Operating Instruction overview. The class was conducted.by a-regional licensing examiner with members of both the Division.of Reactor Safety and Projects and the Division of Radiation Safety and. Safeguards attending.

2. Washinaton Public Power Supply System On November 10, 1992, Region V management and representatives from NRR. s held an enforcement conference with Washington Public Power Supply System management at the Region V office. The subject of the enforcement was the findings of the Augmented Inspection. Team which responded to the August 15, 1992 power oscillation event at WNP-2.
3. Staff from Idaho National Engineering Laboratory (INEL) and COMEX r Corporation-were in Region V on Thursday and Friday, November 12 and 13, i 1992, to conduct emergency = response training in the form of comprehensive severe accident drills. These drills were developed using the RELAP5 engineering simulator capability on the INEL Nuclear Plant Analyzer (NPA). John B. Martin, Regional Administrator led the response team on Thursday afternoon, and B. H. Faulkenberry, Deputy Regional Administrator led the Friday team. A debrief on the exercises is scheduled for Friday afternoon.
