Docket Nos. 50-361 and 50-362 Follow-up Report, Inoperable Fire Doors - April 1985 San Onofre Nuclear Generating Station, Units 2 and 3

Letter, J. G. Haynes (SCE) to J. B. Martin (NRC),

dated April 15, 1985 In accordance with our referenced letter, this submittal provides the follow-up report of fire-rated assemblies (specifically fire doors) found inoperable during routine surveillances in April 1985.

Fire doors found inoperable during the April routine surveillances are identified in the Attachment and include: 2 latching mechanism failures; 0 closure mechanism failures; and 5 doors blocked open typically for construc-tion or maintenance activities. As each of these deficiencies was identified, compensatory firewatches, pursuant to Technical Specification 3.7.9, Action Statement 'a', were established. We are continuing the corrective actions to upgrade door latches and closure mechanisms.

An investigation was conducted to determine the person or persons responsible for the blocked open fire doors. This investigation was unsuccessful in identifying the responsible party in two of the cases.

For the other three cases,Lthe fire doors, which were involved, were recently added to the list of doors requiring surveillance in response to the Updated Fire Hazards Analysis Report. The doors were found blocked open during the time period between the issue of the revised procedure and the posting of the signage identifying these doors as requiring authorization and the posting of an hourly firewatch patrol..To preclude further problems, the fire doors which were added to the surveillance requirement will be clearly marked, with a precaution to get proper authorization prior to blocking open these doors.

' Mr. J. B. Martin Fire doors AC-126 and FH3-304, which were reported inoperable at the end of March 1985, were returned to operability prior to the April 1, 1985 surveillance.

This monthly follow-up report and those sent previously provide updated information associated with an' ongoing condition initially reported in 1983.

The follow-up report covering inoperable fire doors identified in May 1985 will be submitted by June 17,.1985.
