NRC COMMENT 4

During an NRC inspection conducted on May 19,1996 through June 29,1996,a violation of NRC requirements was identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG 1600, the violation is listed below:

Unit 1 Technical Specification 6.8.1 requires that written procedures be established, implemented and maintained, including by reference to Appendix A of Regulatory Guide 1.33, revision 2, procedures for liquid radioactive waste and gaseous effluent systems. Procedure 1-OP-10.1, Operation of the Gas Strippers, revision 16, steps 5.1.6 and 5.3.6, require l

operation of the gas stripper circulation pumps when the gas strippers I

are in service.

Contrary to these requirements, on June 26,1996, both gas stripper l

circulation pumps were secured when both gas strippers were in service, and the gas strippers had been operated in this condition for an i

extended time period.

This is a Severity Level IV violation (Supplement I).

BEPLY IO A NOTICE OF VIOLATION 1.

REASON FOR THE VIOLATION The reason for the violation was due to personnel error which resulted in the failure to follow the requirements of 1-OP-10.1, Operation of Gas Stripper. The operating procedure requires the circulation pumps,1-BR-P-7A/B, to be in service when the gas strippers are processing water. The gas stripper was originally designed to be heated with steam and operated at approximately 2 psig to allow the entrained gases to separate from the liquid. Previously, an evaluation of gas stripper operation was performed which determined that steam could be secured without adversely affecting plant operation and 1-OP-10.1 was revised to permit stripper operation with steam secured. With steam secured, the stripper circulation pumps only recirculate the subcooled water until it is pumped to the appropriate storage tank by the stripper discharge pump. In this mode of operation, the gas stripper was considered to only serve as a surge tank and therefore the recirculation function was viewed as unnecessary by operations personnel.

With steam to the gas strippers removed from service in accordance with 1-OP-10.1, it was inappropriately determined that securing the circulation pumps without procedure guidance was acceptable.

Operations supervision should have instructed personnel to follow the specific requirements of 1-OP-10.1 until an evaluation to eliminate operation of the circulation pumps was completed and a revision to the procedure was approved.

CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED Following the identification that the gas strippers were not being operated in accordance with 1-OP-10.1, a station deviation report was initiated, and the gas stripper circulation pumps were started.

A copy of the deviation report responses from Operations and Engineering were placed in Operations Department Required Reading to continue stressing the importance of procedure compliance.

Operations personnel have been provided reinstruction on the need for procedure compliance and the procedure change process.

CORRECTIVE STEPS WHICH WILL BE TAKEN TO AVOID FURTHER VIOLATIONS Full compliance was achieved when the gas stripper circulation pumps were started and reinstruction on the need for procedure compliance and the procedure change process was provided to operations personnel.

4.

THE DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED Full compliance has been achieved.
