Interim Report - DER 85-07 A 50.55(e) Potentially Reportable Deficiency Relating To The Lack Of Bolt Nut Retention Mechanism On CE Instrument Rack Design File: 85-019-026; D.4.33.2

Reference:

Telephone Conversation between L. Miller and T. Bradish on March 5, 1985

Dear Sir:

The NRC was notified of a potentially reportable deficiency in the ref-erenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due tu the extensive investigation and evaluation required, an Interim Report is attached.

It is now expected that this information will be finalized by May 17, 1985, at which time a complete report will be submitted.

Very truly yours, QC N

INTERIM REPORT - DER 85-07 POTENTIAL REPORTABLE DEFICIENCY ARIZONA NUCLEAR POWER PROJECT PVNGS UNIT 2 & 3 I.

Potential Problem ASME Section III.. Subsection NF, requires a positive means of bolt nut retention. This is lacking on the current CE instrument rack design. Without such a retention mechanism,. installed nuts could become: loose when subjected to vibration. The consequential loosen-ing-of tube clamps and valve / manifold brackets could prevent proper.

operation of associated safety-related instrumentation during seismic events.

II.

Approach To and Status Of Proposed Resolution 3

Bechtel Engineering is currently corresponding with CE on this problem to determine reportability and technical justification for corrective action.

III.

Projected Completion of Corrective Action and Submittal of the Final Report The complete evaluation and final report are forecast to be completed by May 17, 1985.
