The following infomation is reported in accordance with 10 CFR Part 21, Section 21.21 (b)(3) . Northern States, Power Company has determined that components intended for the Monticello Combustible Gas Control System (CGCS) were manufactured with potential defects which may have presented a substantial safety hazard.
The defective components were supplied by the manufacturer of recombiners used in the CGCS. The defects were detected during tt . ting of the components prior to their installation during the 1984 refueling and maintenance outage.
The components were repaired prior to plant startup.
The following information is provided related to this defect:

Director-I&E,..USNRC-MayJ3, 1985 Page'2-f4. Nature of Defect The two circuit breakers had.an internal open connection on one of the phases due_to a missing screw which connects the trip device to the disconnect mechanism.
.The defects were internal to the circuit breaker.and were detected by the NSP. Electrical Construction Testing Group as part of their pre-installation testing.
- 5. Date on Which ! Information Was Obtained
! The' Electrical Construction Testing Group filed a discrepancy report describing the defect on November 24, 1984.
.A Quality' Assurance Nonconformance Report was issued on December 4, 1984 and. Technical Review' Board action was initiated on the following day.
A preliminary investigation report was issued on February,11, l 1985 and submitted'to.the plant staffffor review. ~
The requirements for reportability of this failure were discussed at length and it was unclear whether.or not a Part 21 report was appropriate. The decision was made, however, to interpret the NRC requirements conservatively
, and submit a report. NSP management was notified of this
[ -determination on May 2, 1985.
