Dear Sir:

On March 1,1985, we reported a potential 10CFR50.55(e) item to Region I

[ Reference (b)] . The following information is provided under the interim reporting provision of 10CFR50.55(e)3:

Description of Deficiency Safety-related solenoid valves are used to vent air from safety-related devices, mainly air-operated valves, to position the device to perform a safety function. Some of these solenoid valves are not mounted on the safety-related device and are connected to the device by instrument tubing.

Flexible air hose is used in this connecting tubing to allow for movement of the various portions of the tubing caused by seismic or thermal expansion motion.

Improper site fabrication of the air hose resulted in a partial blockage of the hose by a portion of the lining that was scraped off by the hose fittings. Some lining pieces were hinged so that the blockage was only significant in one air flow direction.

This deficiency was identified during testing of air-operated valves when the valves did not move or moved slowly in one direction or the other, h

Analysis of Safety Implications The ~ potentially defective air hoses were removed before the safety-related devices were tested, so we are unable to determine if the safety functions were ' defeated or if the only effect was a longer response time. The worst case possible would have been the failure of redundant

. containment isolation valves to close.

Corrective Action All installed air hoses will be removed and replaced with hoses that have been properly fabricated or with instrument tubing if the application does not require hose.

Procedures have been developed for proper hose fabrication. Personnel fabricating air hoses have been trained. The hose fabrication process is being covered by our QA program.

A final rep' ort will be submitted when all potentially deficient air hoses have been removed.
