DUQUESNE LIGHT COMPANY Nuclear Power Division Beaver Valley Power Station FITNESS-FOR-DUTY REPORT MRO SIX MONTII REPORT From January 1,1996 through June 30, 1996, our records indicate seven confirmed positives in the Pre-Access program category for the short and long term contractor pools. There was one confirmed positive recorded in the licensee pool in the Random program category.

RANDOM On June 28,1996, a licensee employee experienced a confirmed intoxilyzer positive test result. The employee was placed on indefinite suspension, denied unescorted access and referred to the EAP for evaluation and treatment recommendations. Unescorted access has not been reinstated.

FOLLOW-UP OF PREVIOUS SIX-MONTII REPORT i

FOR-CAUSE OBSERVED BEIIAVIOR l l

On July 13, 1995, a licensee employee experienced a confirmed intoxilyzer positive in the For-Cause, Observed Beha' dor program. This employee had experienced two disciplinary suspension occasions in the previous two years. The employee had returned to work in March,1995, under the condition that he would participate in an evaluation by the EAP and comply with any recommendations resulting from the evaluation. The employee's return to work criteria included drug / alcohol testing at the discretion of Management, which was implemented on July 13, 1995, when he failed to report for work as scheduled.

The employee was placed on indefinite suspension and unescorted access denied on July 13,'1995, for a confirmed positive test result. On July 25,1995, employment was terminated, due to his overall employment record. ,

i An arbitration hearing was held on February 6,1996, with a decision on April 18,1996, l l that the employee be evaluated under the normal procedures for evaluating fitness for return to work and determining the rehabilitation program that he must enter and ,

l 1 l successfully complete. l l

l On May 24,1996, the employee was evaluated by the EAP and referred to Gateway l l

Rehabilitation Center. The employee initiated an intensive outpatient evening program i on June 3,1996, and returned to work on June 10,1996. The employee has been placed i 1

l in a follow-up program and, to date, has successfully completed a pre-access test and one follow-up test. Unescorted access has not been reinstated.
