McGuire Nuclear Station Reply to a Notice of Violation Violation 50-369.370/96-04-01 Technical Specification 6.8.1 requires that written procedures be established, implemented and maintained covering the activities in the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Contrary to the above, on April 28,1996, the licensee failed to adequately implement the requirements of Procedure OP/2/A/6100/SU-1, Mode 6 and Core Alterations Checklist. This procedure requires all Performance Tests required for Mode 6 to be within their surveillance interval. This procedure was completed for Mode 6 entry; however, Performance Test PT/2/A/4350/03A, Electrical Power Source Alignment Verification had not been performed as required.

This is a Severity Level IV violation (Supplement I).

Reolv to Violation 50-369.370/96-04-01 1.

Reason for the violation:

The reason for the violation is an inadequate procedure. Operations Management Procedure (OMP 5-10) Routine Task List does not require a Technical Specification Action item List (TSAIL) entry to track deferred Technical Specification surveillances. In addition, OMP 5-10 does not specifical!y require that the Operations Periodic Test (PT) Group be notified when a periodic test scheduled by the Routine Task List is deferred.

Corrective steos that have been taken and the results achieved:

Upon discovery that PT/2/A/4350/03A, Electrical Power Source Alignment Verification, had not been performed prior to entry into Mode 6, fuel handling activities were immediately suspended and the required surveillance was performed.

A detailed review of documented plant activities identified that the intended Technical Specification surveillance requirements were met by other verifications and signoffs. This review 1

confirmed that the power source requirements for Mode 6 as required by Technical Specifications were available.

No similar events have occurred since implementation of this corrective action.

Corrective steos that will be taken to avoid further violations:

a) Operations personnel will review the periodic test scheduling process to determine if changes are needed to ensure surveillances are completed. This corrective action will be completed by October 26,1996.

McGuire Nuclear Station Floply to a Notice of Violation b) Operations and Engineering personnel will review procedures that control mode changes to determine if procedure revisions are needed to ensure surveillances are completed. This corrective action will be completed by October 26,1996.

c) Procedure OP/2/A/6100/SU-1, Mode 6 and Core Alterations Checklist, will be reviewed to determine if additional Mode 6 periodic tests should be added. This corrective action will be completed by October 26,1996.

d) Operations Management Procedure 5-10, Routine Task List, will be revised to require notification of the Operations Periodic Test Group and require TSAll tracking for any deferred periodic test. This corrective action will be completed by October 26,1996.
