The specification required grease designated as Mobilux 2 to be shipped while the actual shipment contained Mobilux EP-2 grease.

Mobilux 2 and Mobilux EP-2 are lithium soap based greases of the same grade.

EP-2 also contains an additive that makes it suitable for use in " Extreme Pressure" applications. Based on the industrial products brochure describing the Mobilux 2 and the Mobilux EP-2 lubricants and discussions with Mobil Oil personnel, we have concluded that in all cases where Mobilux 2 grease is specified, Hobilux EP-2 may be substituted.

The attached letter from Rosemont to our Fu tchasing Department identifies a potential problem with 1153 series 3 transaitters shipped after January 10, 1984. A previous letter, dated September 1 0, 1984 specified that there is a possibility that a leak path through the ti ansmitter housing could cause the instrument to stop functioning during posti Llated accident conditions. Please verify that nonconforming material reports have been initiated for those 1153 series 3 transmitters received via th a referenced purchase orders.

Rosemount-October 2. 1984 Mr. V.A. Anderson. Director of Purchasing Consumers Power Company 212 West Michigan Avenue Jackson. MI 49201

Subject:

10 CFR.Part 21 Notification, Letter dated September 10. 1984

Reference:

1) Your Purchase order Number 7220-J-245-AC
2) Rosemount Order Number 410965-Gentlemen:

This is a follow-up to the above notification which will provide you with the serial numbers / tag numbers of those transmitters that may be affected by this potential problem. The enclosed listing identifies the 1153 Series 8 transmitters which were manufactured and shipped after January 10, 1984.

Again, the notification applies to only those 1153 Series B transmitters manufactured after January 10. 1984, and those units installed in an environment subject to a LOCA or HEL8.

Currently we are in an extensive evaluation and test program to determine the cause of the leak path. Again, once the cause is identified, a corrective action will be established.

We expect the results of our testing to be available in approximately two weeks. Another status letter will be sent to you at that time.

Very truly yours.

Dear Sir:

In accordance with the requirununts of Title 10,* Chapter 10, Code of Federal tiens, Part 21. Transamerica Delaval, Inc., here-by notifies Canadssion of a potential defect in a compcriant of a DSR or DSRV Sts Ly Diesel Generator. 1here could esist a potan-l tial problem with the Air Filter in the Engine Centrol Panel.

l

__ At Cleveland zlacerie Perry sel.=r statim,_ic_was r r-ely_ dis -

covered that the Air Filter for the Engine Control Panel had a poly d s ete transparent bowl. On Novenbar 5,1978 the Filter purchman -ifie= tim as chanpd from the polyc.fsste trans-t bowl to a metal bowl. Tm polyCwete bowl is #s:srated psig at 125'F and the metal bowl is rated 250 s at 175'F.

Wien the specification was originally issued, the rated the carbonate bowl at 250 psi. The air pressure which the Filter see in operation cycles between 200 and 250 at room tsuperature.

New metal bowl Filters have been sent to Cleveland Electric to re-place the polyc.fwste bowls.

We will send copies of this letter to all sites diere we furnished the Engine Cetrol Panel with a sindliar Filter so that the sites can verify that the Filter has a antal bowl.

Transamerica Delavel has supplied the DSR and DSRV Engine with Digine Conrrol Panels to the following sites:

13 NONCONFORMANCE It was determined that soil boring logs B1, B2, B3 and B4 may be invalid because of an error that occurred during the submittal of boring data obtained by the subcontractor-Raymond Company. This data was used in the evaluation that determined the potential for soil liquefaction of the sand layer at elevations 600.5' and 603', which is now determined to be inconclusive. DIESEL FUEL OIL TANK SOILS BORINGS FILE: 0.4.9.104 SERIAL: 32097

References:

1) Letter, R J Erhardt to H R Denton, Serial 32090, dated November 21, 1984
2) Letter, J E Brunner to ASLB Members, dated December 6, 1984 3)

'.,etter, J A Mooney to H R Denton, Serial 16881, dated May 3, 1982 On November 21, 1984, Consumers Power Cotepany (CP Co) notified your staff of a reportable condition concerning records of soils Borings B-1, B-2, B-3 and B-4.

References 1 and 2 provided additional information on this subject.

Pending resolution of our request to waive the requirement to submit a written 30-day 50.55(e) report during the project shutdown, we have prepared this interim report on the above subject.

t Description of Concern j

It has bee.n determined that the data for Borings B-1, B-2, P-3 and B-4 submitted to the NRC by references 3 and 4, presented in the Midland FSAR and in the response to 50.54(f) questions, do not represent the actual borings which were taken at the diesel generator fuel oil tank location.

Reference 2 is attached to provide a description of the situation and the concern raised as to the potential effect on analysic of liquefaction potential of the soils beneath the diesel fuel oil tanks.

Safety Impset The sole technical issue potentially affected by the invalidity of the 3

previously submitted boring logs B-1 through B-4 relates to the potential

84-17 #1 liquefaction of the sand layer between elevations 600.5' and 603' indicated in boring DF-5.

It is our conclusion based on a conservative assessment of

~

available information that the analysis of the liquefaction potential of the soils beneath the diesel fuel oil tanks is inconclusive.

Root Cause It appears that a combination of events occurred which allowed the use.of incorrect boring logs.

1.

In June and July of 1977 respectively, the same company (Raymond) made the actual diesel fuel oil tank borings and the borings which were erroneously submitted. The borings which were mistakenly submitted related to an AT&T Project in Midland County that had no relationship to Bechtel or Consumers Power Company. The actual B-1 through B-4 borings at the Midisad jobsite were taken to obtain soil samples at the proposed location of the diesel fuel oil tanks.

2.

Both oring series were labeled B-1 through B-4 and the soil types, boring depths and taking of Shelby tube samples for the AT&T borings were, in general, similar to other borings taken at the Midland site. The AT&T borings listed the job address as Midland, Michigan.

3.

During the Site-wide soils investigation in the spring of 1979, it was determined that boring data was required in the area of the diesel generator fuel oil tanks. Bechtel personnel then recalled that borings had been made there in the summer of 1977. The Bechtel Geotechnical Engineering Group (Geotech), which was not involved in the 1977 borings, asked Project Engineering to provide the boring logs. Project Engineering believed that no drilling logs had been made for the B-1 through B-4 borings at the Midland Site. However, their response to Geotech apparently did not clearly convey this but rather it was understood by Geotech that Project Engineering didn't have any loss and not that they didn't exist.

Bechtel speculates that the handwritten drilling logs taken for the AT&T project may have been supplied by the drilling contractor (Raymond) in response to a request by Geotech for logs of the B-1 through B-4 borings taken at the Midland jobsite.

4.

In March of 1982 the handwritten B-1 through B-4 drilling logs again surfaced in conjunction with the design of the site dewatering system.

The recent Bechtel investigation indicates that one Bechtel employee believed that the handwritten driller's logs did not record borings taken in the area of the diesel fuel oil storage tanks and that there may have been discussions within Bechtel's Geotech group on this matter. Apparently a decision was made based on the available data to include these driller's logs as input to the liquefaction analysis for the buried diesel fuel oil Subsequently, Consumers Power submitted the handwritten driller's tanks.

loss to NRC informally and then via reference 3.

Bechtel prepared typewritten boring loss from the handwritten driller's 5.

logs for inclusion in the June 1982 FSAR submittal. This limited further opportunity to detect from the original driller's logs that they were incorrect nes set. nsnca. won?

Q-00011 page 5 of 7 3

Serial 32097 i

84-17 #1 as to the Midland site location since the original driller's logs were nov back in the files. The typed boring los profiles include additional information 1

which is believed to have been obtained from the test records of the borings actually taken at the Midland jobsite, such as the boring coordinates.

It is normal practice for a handwr'itten boring los to be produced at the time a boring is made. These logs are produced by the technical person overseeing the drilling operation. Information concerning boring locations is entered in the logs by such technical personnel, and thus can be directly traced to the Midland jobsite. In the case of Borings B-1 through B-4, the erroneous loss (ie, the loss of the AT&T borings) contained no location coordinates or other information directly indicating the location of the borings. Instead, the location information obtained from testing data concerning the onsite B-1 through B-4 borings was assumed to be applicable to the logs of the offsite i

borings. As previously stated, this confusion resulted in logs of the incorrect borings being submitted to the NRC. Ihe absence of location information on the drilling loss traceable to the Midland site is a l

circumstance unique -to Borings-B-1 through B-4.---

A review of all Bechtel borings incuded in Section D.1.4 of the Soils 50.54(f) response indicates that borings B-1 through B-4 are the only borings made from a driller's log. All the other borings were produced from handwritten boring logs by a Bechtel Geotechnical Engineer assigned to the drilling. The normal practice of involvement by Geotech personnel would preclude any generic repetition of the unique circumstances of this particular situation.

Corrective Action The controls in place at the time of the Project shutdown were adequate to prevent a recurrence of this problem, thus no process corrective action is necessary.

Part corrective action required includes correcting the affected documentation

~ and determining if additional analysis or additional field information is necessary to address the issue of potential liquefaction of soils below the l

diesel fuel oil tanks.

Because of the Project shutdown, these corrective actions will not be pursued and no further reports on this subject will be made unless the Midland Energy Center is reactivated. This item will be entered in our Correspondence and Commitment Management System for long-ters tracking.

10555 Sir:

- - -In - accordance-wi th - the-requi rements -of-10-CFR -pa rt-21 r Ruskin-Ma nuf acturi ng Company, located at 3900 Dr. Greaves Road, Grandview, Missouri 64030, is herewith reporting that we have discovered a deficiency with equipment furnished by Ruskin Manufacturing Company for Nuclear Power Plant application.

On August 7, 1984 Ruskin received a letter from The Waldinger Corporation addressing seven (7) fire dampers failure to close under normal duct pressure at Palo Verde Nuclear Generating Station.

On August 13 1984 Ruskin visited the jobsite to inspect dampers. The results of the inspection revealed damper condition to exhibit bent blades, bent blade tracks, rust and zine rich paint on springs.

On August 20, 1984 after Engineering review of inspection results, Ruskin again visited the jobsite for Engineering review of above referenced damper conditions.

As a result of this review Ruskin recommended that the blade packages be replaced in' the affected fire dampers.

On October 30, 1984, Ruskin was notified, by The Waldinger Corporation, that the new blade packages had been installed and tested.

The fire dampers with the newly installed blade packages would still not cycle under normal duct pressure.

Ruskin Engineering immediately began to evaluate the problem to determine the cause of the dampers failure to close under~ air flow.

Results of this evaluation.

indicated the test methods originally utilized to test dampers under flow may not accurately depict actual installed conditions.

Ruskin fire dampers were tested in accordance with AMCA 500, test figure 5.5, mounted on the face of the test tunnel. Actual installed condition, in ductwork, may not result in proper performance as tested to AMCA.

Ruskin, on November 2, and 51984, performed air flow tests in accordance with AMCA 500 with damper mounted on face of tunnel as well as with damper mounted in ductwork.

The results of this test indicated that face mounted damper would close at much higher static i

l pressure than the damper which was installed in'd6ctwork.

- e Q-00012 pass 4 of 7' November 6, 1984 U. S. Nuclear Regulatory Commission Page 2 Since no facility exists to properly test dampers under flow that would simulate all field installed conditions, and relationship between face mounted damper and damper installed in doctwork cannot be accurately calculated, Ruskin recommends that fire dampers supplied with closure springs, which require closure 'under air flow conditions, be tested to verify proper operation. This condition affects only interlocking blade fire dampers Model 180-21. IBD-23 and NIBD-23 supplied with closure springs by Ruskin.

Ruskin is currently researching methods to modify fire dampers to ensure closure wider air flow.

Attachment I to this letter identifies the affected jobsites by location and the number of units supplied to each.

During the course of a Vibration Aging and Seismic Testing Program of an EFCO-600 Series 600 Actuator manufactured by Gulf & Western Fluid Systems Division, a latch roller bearing failure was observed. The bearing is the latching roller located on the blocking lever mechnism. These tests were being conducted under a contract from Stone & Webster Engineering Corporation, the designated Architect / Engineer for Niagara Mohawk Power Corporation' for the Nine Mile Point Plant. The EFCO-600 Series Actuators are quarter turn actuators used to actuate (on/off) main steam isolation valves (MSIV) that were manufactured by Gulf & Western Fluid Systems Division formerly of 25 Graystone Avenue, Warwick, El 02886.

Subsequent to the bearing failure a static load test was conducted on an identical bearing installed in the EFC0 Series 600 Actuator to determine the actual static load on the bearing. These static load tests were conducted at the Crosby Valve & Gage Company, Wrentham, Massachusetts, under contract to Stone & Webster. These tests showed that the static loads imposed on the bearing exceeded the bearing manufacturer's==vf=um published load for that bearing.. (Torrington Bearing Design Number 28NBL4855YJ.)

Since failure of the bearing in service would render the main steam i.

isolation valve inoperative, Crosby Valve & Gage Company and Gulf & Western Manufacturing Company have determined that, in their opinion, a probable cause for reporting under 10CFR21 exists.

Dear Mr. Anderson:

7 g g o A

pyg y o.rg We understand that your facility purchased four (4) EFCO-600 Main Steam Isolation Valve Actuators in 1978-79.

Crosby Valve &

Gage Company

(" Crosby"),

which purchased the Fluid Systems Division from Gulf & Western Manufacturing Company in 1983, has advised us that a Vibration Aging and Seismic Testing Program conducted on a similar Actuator has resulted in the failure of the Actuator's latch roller bearing No. 28N8L48554J.

The load test results are summarized below:

1.

Strainsert i Q7448-1 A.

4-Runs B.

Average Force Equals 103,670 Lbs.

2.

Strainsert f Q7448-2 A.

4-Runs i

B.

Average Force Equals 104,465 Lbs.

The purpose' of this letter is to notiify you that the latch roller bearings contained in the EFCO-600 Main Steam Isolation Valve Actuators sold to your facility. are identical to the bearing tested by Crosby, and to alert you to the possibility that the latch roller bearings in your Actuators may be subject to failure under similar test conditions.

i Crosby has advised us that it has determined that the hydraulic latching pressure of your Actuator is approximately the same-as that of the test Actuator, and therefore the static load imposed on the present bearing exceeds the bearing manufacturer's published maximum rated load for that bearing.

Page Two February 13, 1985 EFCO-600 MAIN STEAM ISOLATION VALVE ACTUATO_R_

Crosby is working on the design of a new bearing for this application.

Notification will be issued upon completion of this design effort.

In the meantime it is strongly reconenended that none of the EFCO-600 Main Steam Isolation Valve Actuators in your pe: session be installed, or, if already installed, that due consideration be given to the effects on the nuclear steam supply system should the bearing fail and the valve become inoperative.

Crosby has agreed to notify the Nuclear Regulatory Comunission pursuant to 10CFR 521.

Any questions you have concerning the points raised in esis~

letter should be directed to:
