' Event' Description
'A review of previous. performances 'of Surveillance Instruction (SI) 241,
" Visual Inspection of Spray and/or Sprinkler. System Spray Headers, Nozzles,
.and Nozzle Spray Area", was performed on December 18, 1984. This was in conjunction with a'special performance of this instruction to address 10_CFR 50, Appendix R related sprinkler system deficiencies. As a result of.this review, it was' discovered that the last complete scheduled performance of this instruction, which implements Technical Specification Surveillance ' Requirements 4.7.11.2.c.2 and 4.7.11.2.c.8 for both units 1
-and'2, was last completed on December 28, 1982. The 18 month performance
~ frequency (plus allowable extension) for these Technical Specifications surveillance requirements was exceeded by approximately 45 days.
LCause-The cause of this event was a failure to issue a Surveillance Instruction 4 performance package for routine performance when due. A conditional refueling outage performance-of this SI was. issued and performed during the unit 1,
. cycle 2 refueling outage in March 1984. The only areas inspected at that time were the reactor coolant. pump spray headers inside unit 1 containment.
- Following performance as intended, the instruction package was returned to Plant Services. The Plant Services staff assumed the SI work package was
'a complete performance.- On this assumption, the next routine performance
- was scheduled for September 1985, eliminating any issue of a routine per-formance' package when due.in June 1984. A list of all 18 month and refueling outage: frequency sis for unit 0, units 1 and 2, and unit 2 was sent to each responsible section for' potential SI scheduling prior to the unit 2,. cycle 2 refueling outage., SI-241'was not scheduled by'the responsible section in' response to.this list.

Areas provided,with automatic' sprinkler systems, which contain redundant' systems or components which could be damaged,1were inspected during the previously mentioned special' performance.of SI-241. The provision of
' continuous firewatch coverage, according.to Technical Specification Action Statement 3.7.11.2.a.:was'therefore not applicable. ~ For other areas where S

~ Lthe involved surveillance requirement has not been performed, the automatic
- sprinkler system protection was declared ~ inoperable... For these areas, it was verified.that previously established hourly firewatch coverage was 1
sin compliance with the required action statement, and that backup fire suppression equipment.was operable.
At ~1 east 70 percent of-the sprinkler and spray.' system headers, nozzles, and 1 sprinkler heads have been inspected on the previously referenced special'
' performance of SI-241..The' remainder which require inspection will be
' inspected, according to,the applicable surveillance requirement, by January, 25,~1985.

-2 SEQUOYAH NUCLEAR PLANT-SPECIAL REPORT 84-09 UNITS 1 AND 2 To prevent an occurrence of this type in the future,-all special performances and refueling outage performances of surveillance instructions will state if this SI performance was a complete performance of the SI, and if this performance date is to be used to schedule the next performance.
SI-241 has been split into three separate, independently scheduled sis. One is for inside unit 1 containment, one is for inside unit 2 containment, and one is for outside the containment buildings.
