Catawba Nuclear Station Unit 2 IDENTIFICATION OF DEFICIENCY:

. A total of twenty-seven 14 and 16 NA1 Rotork Electric Motor Operators were reported'to Duke by Rotork as possibly having nitrile thrust base 0-rings.

The thrust base 0-ring material should have been viton. The deficiency was identified on August 3,1984.

INITIAL REPORT:

were notified of the subject deficiency by W. O. Henry, T. L. Utterback, R. L. Oakley and J. H. Henkel of Duke Power Company, Charlotte, NC 28242.

COMPONENT AND/OR SUPPLIER:

notork Model 14NA1 and-16NA1 operators.

DESCRIPTION OF DEFICIENCY:

The twenty-seven operators supplied with nitrile thrust base 0-rings do not

- comply with the' environmental qualification report for the inside-containment model NA1 operator. The qualification report is based on tests of operators J.

using viton thrust base 0-rings. Nine operators are currently assigned to inside-and outside-containment applications (the remaining 18 are gpare), where environments can ' reach -3300F and ' radiation levels of 1.1x100 rads total Integrated dose.. The vitgn 0-rings have been tested to temperatures of 3400F and radiation levels of 2x100 rads in the thrust base application. The nitrile 0-rings have 3

not been tested to these conditions. From material publications, the nitrile material is -rated for only 2000F and lx107 rads.

. ANALYSIS OF SAFETY lHPLICATIONS:

Failure of these 0-rings would result in a loss of gear train oil, and subsequent loss of valve / operator function.

Operators furnished with nitrile 0-rings have not been qualified by environmental testing:and therefore cannot be relied upon to assure operability.

the FSAR' requires that the operators for active valves be qualified by environmental qualification.

CORRECTIVE ACTION:

All affected operators were to be returned to Rotork for enange-out of the 0-rings. Upon further investigation, Rotork has been able to determine at what point in time the use of nit = ie 0-rings took place. Rotork has determined that operators shipped pr3..r to December 1983 have the correct viton10-rings. All operators in question at Catawba were shipped prior to December 1983 and therefore will not-be returned to Rotork.

Operators.at McGuire shipped af ter December 1983 have been returned to Rotork and repaired.
