50-328 - FACILITY OPERATING LICENSE DPR SPECIAL REPORT 84-10 The enclosed licensee event report provides details concerning the inoperability of a fire door in excess of seven days. This event is reported in accordance with Sequoyah Unit 2 Technical Specification 3.7.12 and 6.9.2.

Event Description and Probable Cause Fire ' door A-77'was open without a fire barrier breaching permit and the required roving fire watch. This door is located on elevation 690 between the containment airlock doors and the containment purge air exhaust filters and fans. Unit 2 was in a refueling outage, and door A-77'is located in the passageway normally used for access co the annulus and lower containment.
A large number of personnel pass throu6h this door each day to perform outage work.. This personnel traffic is scattered throughout the day, and any fire would probably have been detected. Also, Health Physics perronnel are stationed near this door to collect contaminated tools being removed from containment, therefore, providing continuous personnel coverage of the area.
Corrective Action-A roving fire watch was established per Technical Specification 3.7.12 on December 1, 1984. A breaching permit was issued immediately af ter the door was discovered to be open without proper fire watch in effect. - A permit will be maintained on the door as required during future outages.
