This is in response to the Notice of Violation cited in Inspection No. 50-412/84-18 and listed in Appendix A (Notice of Violation) attached to your letter to Mr. E. J. Woolever, dated January 28, 1985.
Notice of Violation 10CFR50, Appendix B, Criterion V, requires that activities affecting quality shall be prescribed by procedures and shall be accomplished in accordance with these procedures.
Specification 2BVS-981 requires Level "C" storage to be maintained for reactor coolant system piping and valves to protect this equipment from exposure to the environment and airborne contaminants.
Contrary to the above, certain piping and valve items in a 3-inch reac-tor coolant system bypass line for the "B" loop were not properly sealed to protect this equipment from exposure to the environment and airborne contaminants.
Response
Immediate Action Taken Following identification of the deficiency, the 3" gate valve was recovered and resealed to prevent further contamination. Nonconformance
& Disposition (N&D) 7886 was initiated December 13, 1984, to address the cleanness violation.
The disposition required that all potentially contaminated areas be inspected and any contaminants be removed from the g3180087850304 GI i

valve internals, internal surfaces be restored to their original " clean" condition, and that any damaged areas be reported to the Engineers for further action.
The dispositon also required that orifice openings on the orifice flanges be inspected for damage and any damage be reported to Engineering - for further action.
It also required that the orifice openings be inspected for the existence of debris and any debris to be removed and _-the openings-to be covered when restoration was completed.
These actions were completed February 14, 1985.. As part of the site
. post-turnover procedures, this system (RCS-6) will receive a once-thrcagh flush, which will further ensure that there is no debris remaining in the line.

. Reassembly of the valve is scheduled for the first quarter of 1985.
Long Term Corrective Action DLC has proceeded with the following to preclude reoccurance of storage /
maintenance problems:
.l..The -establishment of a Composite Review Work Group (CRWG) which has as.its objectives (a) identifying any instances where equipment maintenance requirements are not clearly specified or not practical to implement, and (b) identifying any noncompliances which may exist within permanent plant buildings / areas with respect to the in-place storage and maintenance requirements of Sepcification No. 2BVS-981 for Cat. I equipment only.
Both' satisfactory and unsatisfactory conditions identified by the CRWG_ will be documented via the use of Site Quality Control (SQC)
Inspection Reports. The CRWG began its inspection efforts on January
/, 1985, and is expected to complete its effort by April 1986.
To date, the inspection has covered approximately $50 equipment-items of which nine (9) unsatisfactory conditions are currently outstanding.
- 2. To provide management accountability, a Management Oversight Commit-tee (MOC) will _ be formed to reveiw the report published by the CRWG and Management Committee for Storage Review (MCSR) to assess the overall program effectiveness, and to recommend program changes or other actions consistent with prevailing project conditions. The MDC will draw its membership from the A/E (Stone & Webster Construction and Engineering), the two prime subcontractors Sargeant Electric and Schneider Power, and DLC SQC.
It will begin functioning in February, 1985..
