O' Re illy SPR 84-014 October 29, 1984 CAUSE OF INOPERABILITY On September 30, 1984, the Plant was in the process of cooling down for entry into Mode V for the First Refueling Outage.

When the Reactor Coolant System (RCS) temperature reached 300*F, Technical Specification 3.4.9.3, " Overpressure Protection Systems," required either:

a) two (2) operable Power Operated Relief Valves (PORVs), or b) the RCS depressurized with a vent of greater than or equal to 2.7 rTuare inches.

The operability of the two PORVs ensures that the RCS will be protected from pressure transients which would exceed the limits of Appendix G to 10CFR50.

The Licensee was unable to meet the above requirement for two (2) operable PORVs because PORV PCV-445A had previously been declared inoperable and blocked because of a design error.

The details of this design error are addressed in LER 83-100, Revision 1, dated January 27, 1984.

PLANS FOR RESTORING TO OPERABLE STATUS South Carolina Electric and Gas Company has received an amendment to Technical Specification 3.4.9.3 allowing the use of.the Residual Heat Removal System (RHRS) relief valves as the primary method of mitigating cold overpressurization transients.

The hardware modifications for the transition from PORVs to RHRS relief valves for Cold Overpressure Protection System (COPS) will be completed during the present refueling outage enabling compliance with revised Technical Specification 3.4.9.3.
