This documents my report by telephot.- under the provisions of 10CFR21 on October 24, 1984, at approximately 5:20 p.m.

Individual Who Reported Mr. W. F. Sailer, Vice President and General Manager, Quality Assurance Division, Gilbert /Conunonwealth, Inc.

Facility Involved Perry Nuclear Power Plant, Units 1 and 2, being constructed by the Cleveland Electric Illuminating Company.

Activity / Component Involved Feedwater System pipe rupture design.

Nature of the Defect An erroneous asssumption was made in desi F "ing for postulated pipe rupture of the feedwater system. It was assumed th t jet force and jet shape information used for the feedwater pump side o^ a break could also be used for the reactor side of the break.

A recently received report of GE's analysis indicates that the total jet force is 25% greater, and the jet shape is potentially different, than had 1

been assumed in the G/C calculation. This may result in the failure of protection devices or essential equipment.

  • Date Information Obtained The QAD General Manager was-informed on October 24, 1984 of the reportability of this item.

The jet pressure calculations for other Perry systems have been reviewed. It has been. determined that this discrepancy exists only in the Feedwater System.

Corrective Action Gilbert / Commonwealth will review all targets and shields affected by the increased pressure to determine whether failure of the protection devise or

.the essential equipment will occur. .

The review will take three months to complete. Additional shielding will be designed and installed if required by the results of the review.
