In accordance with our referenced letter, this. submittal prcvides the follow-up report of fire-rated assemblies (specifically Mre doors) found inoperable during routine surveillances in September 1984.

Fire ~ doors found inoperable during the September routine surveillances are identified in the Attachment and include: 5 latching mechanism failures; 14 closure mechanism failures; and 2 doors blocked open typically for construction or maintenance activities. An investigation was conducted to determine the ' person or persons responsible for the blocked open fire doors, however, results were inconclusive. As each of these deficiencies was identified, compensatory fire watches, pursuant to Technical Specification 3.7.9, Action Statement 'a', were established.

We are continuing the corrective actions to upgrade door latches and closure mechanisms. A disciplinary action policy was implemented August 15, 1984, by Station Management, in order to eliminate occurrences of blocked open fire doors without the required Fire Barrier Removal and Reinstallation Request.

This monthly follow-up report and those sent previously provide updated information' associated with an ongoing condition initially reported in 1983. The folicw-up report covering inoperable fire doors identified in October 1984 will be submitted by Novenber 21, 1984.
