UNIT 2 DOCKET NO. 50 446 BORG WARNER / INTERNATIONAL PRODUCTS (BW/IP) CHECK VALVES
Dear Mr. Yandell:
In early July, a BW/IP check valve failed during Unit 2 preoperational testing.
TV Electric has concluded that this failure was not related to the previous failures of BW/IP check valves.
An engineering report which describes the condition and its analysis is enclosed for your information.
In summary. the engineering report describes a unique set of conditions which in Combination caused the check valve failure.
These ualque conditiens were an Auxiliary Feedwater System (AFW) system configuration which caused an excessive flowrote. check valve modifications (Unit 2 only),
and system effects due to a flow restricting orifice.
The corrective actions to preclude recurrence include revision of the AFW operation procedures to eliminate the possibility of excessive flow-rates removal of the configuration difference (i.e., the back stop). and acoustic monitoring to determine orifice effects.
