dated January 15, 1985 In accordance with our referenced letter, this submittal provides the follow-up report of fire-rated assemblies (specifically fire doors) found inoperable during routine surveillances in January 1985.

Fire doors found inoperable during the January routine surveillances are  !

identified in the Attachment and include: 15 latching mechanism failuresj 3 closure mechanism failures; and 5 doors blocked open typically for construction or maintenance activities. An investigation was conducted to determine the person or persons responsible for the blocked open fire doors, ,

but the investigation was unsuccessful in identifying the responsible party.

As each of these deficiencies was identified, compensatory fire watches, pursuant to Technical Specification 3.7.9, Action Statement 'a', were established. We also are continuing the corrective actions to upgrade door latches and closure mechanisms. l This monthly follow-up report and those sent previously provide updated  !

information associated with an ongoing condition initially reported in 1983. The follow-up report covering inoperable fire doors identified in February 1985 will be submitted by March 20, 1985.
