20555 Operating License DPR-58/74 Docket Nos. 50-315/50-316 Document Control Manager:
This special report is being submitted pursuant to the requirements of Technical Specification 3.7.10 Action Statement (a) to inform you of Fire Rated Assemblies (Fire Doors) being inoperable in excess of seven days. The inoperable fire door events are all associated with the installation of a design change which upgrades fire doors as part of our Appendix "R" commitments. A total of 59 fire doors are affected by this design change.
During the modifications,27 of the doors remained inoperable in excess of seven days. The cause for this can be attributed to one or more of the following reasons-o On several doors the hardware from the existing doors had been designated for reinstallation on the new doors but for various reasons could not be used or required extensive modifications.
o New fire door assemblies did not fit the designated location, thus requiring modifications or replacement.
o Deficiencies were identified during a post-installation inspection by the Underwriters' Laboratory Representative.
Some of these deficiencies were corrected; however, for eleven doors the corrective actions require the procurement of replacement hardware and these doors remain inoperable.
