Docket No. 50-361 Special Report - Seismic Instrumentation San Onofre Nuclear Generating Station, Unit 2 Pursuant to Section 6.9.2 of Appendix A, Technical Specifications to Facility Operating License NPF-10 for San Onofre Unit 2, this Special Report is being submitted as required by Action Statement 'a' associated with Limiting Condition for Operation (LCO) 3.3.3.3. This Action Statement requires that a Special Report be prepared and submitted to +he Commission within 10 days after any of the seismic instrumentation listed in Table 3.3-7 of the Technical Specifications, is inoperable for more than 30 days. The cause of the malfunction and plans for restoring the instrument to operate status, are cutlined below.

On November 3, 1984, with Unit 2 in Mode 6, seismic instrumentation was removed from service for design change work and to perform 18-month surveillance testing. The strong-motion accelerometers 2XT-8020C and D located on the reactor coolant. pump and prenurizer base support, respectively, were declared inaperable when they failed to meet the functional calibration requirements of Procedure 5023-II-2.2. The accelerometers were removed and shipped to the vendor for appropriate calibration and/or repair. The cause of the malfunction is unknown and will be determined by the vendor. Our i investigation was not able to determine when the accelerometers became inoperable because there have been no seismic events during this period.

An update to this Special Report will be submitted addressing the significance of the event as well as the cause of the malfunction and corrective actions.

The instruments are anticipated to be placed back in service withil 30 days of this report.
