Description of Deficiency 91
' Investigation Request IR-37 was initiated to perform a. site inspection of Anchor / Darling (AD) swing check valves for missing tack.
welds per DER 82-33. The following two deficiencies were identified during the course of this investigation.

In valves 1PAFAV005 and IPAFBV022, the' set screw was not tight cM ',,
enough against the.hingerpin" causing a condition wherein the hinge pin is able to slide through the hinge support. AD swing check valves require a set screw to be tack welded to.the hinge
-support to prevent the hinge pin from becoming disengaged from the hinga. support.
2.
In valves 1PCTAV037, 1PCTBV038, 1PCTAV016, and 1PCTBV020, the disc nut pin was not tack welded to the dise nut. The disc nut pin is required to be tack welded to the disc nut to provide a positive attachment of the disc.
Evaluation The root cause of both the~ above conditions is attributed to errors during assembly of the valves prior to delivery to the site. Bechtel discussed the above conditions with AD and both agreed that either

condition:1 or 2 could make the valve inoperable. The valve could M sak.at back flow, disengaged parts could restrict the flow through the valve small and/or small disengar,ed. parts could flow through the e-(
system' causing damage to other equipment.
Had this deficiency remained uncorrected, check valves in various saft ty-related systems including Aniliary Feedwater could have

. fail'ed, thereby precluding r,afety-related systems from performing k
essential functions.

Corrective Action 1.
Attached is the list of AD swing' check. valves used in Unit 1.
aAll the Unit 1 listed valves are being inspected and documented on special CIP 592.0. The defective valves will be reworked per AD's direction and inspected via NCRs SM-5320, SM-5321, SM-5322, and SM-5355.
- 2. ~All CT,-FP, and SP valves in Unit 1 listed in the attachment will be inspected and reworked as required prior to entry into Mode 6.
The remaining' valves will be inspected prior to Mode 4.

Units 2 and 3 will be inspected and reworked as required by IR-48.
Special CIPs 592.0, 606.0, and 607.0 have beea initiated:
to cover the defects (locse hinge pin and missing tack weiding on the disc unit) as identified in Section I of this DER and also the defect (missing tack welding on the set screw) previously identified on DER 82-33. Any subsequent required NCRs shall cross-reference this DER for reportability disposition. This action will be completed prior to operating license for each unit.

The supplier drawings listed below have been revised by issuance of Supplier Document Change Notices (SDCN) to show staking at three places as an alternative to tack welding of the set screw.
