PVNGS UNITS 1, 2, 3 I.

Description of Deficiency Investigation-Request IR-12 was issued to investigate. General.

Electric _(GE) HFA relay's normally closed contact settings at PVNGS.

This-IR was created in response to NRC IE Notice No. 83-19.. The investigation found improper contact settings on several normally-closed HFA relays. These relays were located in Unit 1, Class IE-4.16 KV switchgear and Class 1E 125V DC control centers.

Evaluation-i

.Contact gaps are either preset by GE at the-factory or adjusted by construction when a Design Change Package (DCP) requires changing contacts from normally open to normally closed.. On implementation of DCPs, construction follows the procedure for contact gap changes and e

adjustments.in GE instruction booklets (GEH 2024 for type HFA 51 and GRK 45484 for type HFA 151), therefore, the root cause of contact misadjustment is determined, as incorrect. settings mode in the supplier's factory, as reported in IE Notice No. 83-19.

This improper contact adjustment could cause chattering during a seismic event which may cause malfunction of'other safety-related'

equipment. The evaluation of this DER is being done on a: generic-basis since it is likely that different relay contacts will require adjustment in each unit.,

Based on the above, this condition is evaluated as reportable unier the requirements of 10CFR50.55(e); since, if this condition were to remain uncorrected, it would represent a significant safety

condition. This condition is also evaluated as reportable under

10CFR Part 2, which was reported by GE on July 26. 1982 and also-identified by the NRC with IE Information Notice No. 83-19 on-April 5, 1983.

III.

Corrective Action The relay contacts will be readjusted in accordance with GE.

In Unit 1-Startup Work Authorizations (SWAs) 17763, 17869, 17870,

-17912, 17913, 17961, 17970, 17971, 18049, 18050, 18051, 18063,

-18064, 18227,'18367, 18739,.and 19149 were issued in response to

All Unit 1 Class 1E HFA relays have been readjusted-as required.

Units 2 and 3 HFA relay contact settings will'be inspected and' adjusted if required per IR-012 in the same manner as completed for Unit 1.

These tasks will be completed for Class 1E HFA relays prior to operating license in each unit.
