--4 CATAWBA NUCLEAR STATION REPORT NUMBER: SD 414/84-22 REPORT DATE:
November 21, 1984 FACILITY: Catawba Nuclear Station Unit 2 IDENTIFICATION OF POTENTIAL DEFICIENCY:
The operator on Valve 2KC228B was found to be damaged during i & E checkout of the valve and reported on NCI 19011. Af ter investigation it was determined I
that the operator (S/N B8243-A/1) was the Rotork VNO Wiring design. The Duke external wiring for the valve was set-up for the Rotork NO wiring design.
INITIAL REPORT:
On October 25,1984, A. L. Ignatonis, NRC Region II, Atlanta, GA was notified of potential deficiency by L. M. Coggins, R. L. Oakley, J. H. Henkel and T. L. Utterback of Duke Power Company, Charlotte, NC 28242.
COMPONENT AND/OR SUPPLIER:
Rotork Electric Motor Operators.
DESCRIPTION OF POTENTIAL DEFICIENCY:
The Rotork NO design incorporated a mechanical latch to lock out the torque switches during the first revolutions of the operator when opening and closing the valve. The VNO design incorporates a electrical latch which must be furnished in the external wiring. Using the VNO design in a NO external wiring design can resalt in operator damage.
The NO design operator is no longer furnished by Rotork.
The operator on 2KC228B was determined to a replacement operator for the original NO operator.

ANALYSIS OF SAFETY IMPLICATIONS Failure for 2KC228B could result in cooling flow to essential components being -
reduced to below acceptable levels. The use of VNO operators in NO external applications can result in loss of valve / operator function.
Three other operators have been determined to be Vrio designs possibly in NO external wiring applications on Active Valves (Item 5B-202 S/N B5886-A/1, A/2;93-204, S/N B7543-A/1).
CORRECTIVE ACTION AND STATUS:
At this time the determination of reportability and investigation of other possible applications with other VNO operators in NO external wiring has not been completed. A complete audit of Unit 2 operators is being completed. A final report will be issued by January 1,1985.

The external wiring to 2KC228B will be corrected by January 1,1985.
