Gentlemen :

The purpose of this letter is to confirm in writing the telepho-nic information conveyed to Mr. Young of your office on November 15, 1984, 1.

The name ar.d address of the entity informing the Ccanmission are:

Greer Hydraulics division of VSI Corporation 6500 East Slauson Avenue City of Ccxamerce, CA 90040-3193 The officer is Jon Knartzer.

ii. The basic component which is believed to fail to comply with or contain a defect is an actuator. Greer has been informed that such actuators are used with the water make-up valve and that they are intended for installation at "Seabrook Unit 2" located in New Hampshire.

iii. Greer believes the general contractor is:

United Engineers and Constructors 30 S. 17th Street P.O. Box 8223 Philadelphia, PA 19101

-iv.

The defect is a swelling of the seals within the actuator, in-creasing the maximum closing time above the maximum allowable time of three seconds.

vii Twenty-four (24)..of such actuators have been supplied to:

Posi-Seal' International Rt. 49 and U.S.:95 North Stonington, CT 06359-Greer ' understands that none of such actuators -have been installed

.at.any site.

vii. Samples of such actuators are being. tested by: -

Sterns and Rogers 4500 Cherry Creek Drive Denver, CO 80217 Greer is 'also testing to determine the cause of swelling ofithe seals.

s iix.. It is suspected that the swelling is the result of improper long-term storage or use of improper ' lubricants -~during-the assembly.

-procedures. Af ter determination ' of the cause,! the seals will be re-placed and the actuators will be. reassembled and re-tested. -Such-repairs will be completed within 60 days.

ix.

Mvice to purchasers will depend upon determination of the pri-mary cause of the failure.
