Dear Dr. Murley:

This Special Report, required by Technical Specification 4.7 3.d, documents eight (8) occurrences of injection of water into the Reactor Coolant System (RCS) of Unit 2 due to Reactor Core isolation Cooling (RClC).svstem act-uations.

On August 4, 1984, two (2) injections occurred during the performance of a Hot Functional test to tune up the RCIC Turbine Control System.

On August 5,1984, two (2) Injections occurred as part of Startup Test (ST) 14.2, ' Reactor Vessel Injection'. One (1) Injection occurred on August 7, 1984, following ST 31.1 ' Loss of Turbine Generator and Of f-site Power'.

ST 14.3, ' Rated Pressure Auto quick Start to Vessel' included another injection into the RCS on August 18, 1984. ST 14.3 was run again on August 21, 1984, resulting in another injection. On September 3,1984, an injection occurred as part of ST 14.4, ' Low Pressure Auto quick Start to Vessel'.

As of September 8,1984, the total number of RCIC actuation cycles is twenty-six (26). A conservative analysis indicates the current usage factor for the af fected nozzles is less than 0.70.
