out until 1225 CST on 10/12/84.

Probable Cause The'. expiration date at thettop oflthe.Penetratica Fire ~ Barrier Breaching.

Permit (PFBBP)' was in conflict with the date and time the breach was

~ ~

actually made. .This caused confusion on the correct return-to-normal date.

9 Corrective Action ~

The ' expired permit was dicussed with the personnel involved, and the.

Operations Section Assistant Shift Engineers have been directed to perform a daily audit .of; the PFBBP book. . In addition, training ~

letters have been distributed to fire watch personnel.
