SEQUOYAH NUCLEAR PLANT UNIT 1 SPECIAL REPORT 84-04 ve Event Description and Probable Cause A penetration fire barrier breaching permit was issued on 08/16/84 for fire door A-33 due to the door panel splitting at the welded seam. This door is located on elevation 669 between the refueling water purification
- pump and filter area (safety related area) and the cask decon collector tank, pump, and filter area (non-safety related) . The cause of the split panel has been attributed to hard closing forces due to the high differential pressure across the door.
Corrective Actions A roving fire watch was established per Technical Specification 3.7.12 on 08/16/84 when the permit was issued. .A new door panel was received on 09/10/84 and installation of the new panel was completed on 09/12/84.
