Docket No. 50-414 Special' Report-Vibration and Loose Parts Monitoring System On August 17, 1995, Unit 2 Loose Parts Monitoring (LPM) System Channel 12 (Steam Generator [S/G)

D' Channel 2) was declared inoperable due to a channel sensor failing the acceptance criteria during the performance of PT/0/A/4600/03 (LPM Spectrum Comparison).

This. failure was attributed to a loose sensor mount.

Another loose ' parts sensor on this S/G is functional and continues to provide

. coverage.

The present LPH System is scheduled to be replaced during the upcoming. Unit 2 outage, 2EOC7. Problem Investigation. Process (PIP) 2-C95-1255 was initiated and'is the tracking document for this failure.

Since this channel has been inoperable for 30 days; it is reportable pursuant to Technical Specification 3.3.3.9.
