Approximately 10% of the switches purchased for test switches in the protection / safeguard circuitry did not pass installation continuity checks.

Test switch is placed in the open position to allow test of circuit.

Failure of switch to pass signal in normally closed position would prevent actuation of the respective protection / safeguard signal.

Manufacturer indicated this appears to be an isolate'd problem and is inves -

tigating.

s Corrective Action Which Has Been Taken:

1.

Reverified continuity of REES test switches which passed continuity checks and were installed.

2.

Disassembled switches not passing continuity check and observed that contacts appeared deformed.

3.

Disassembled switches that passed continuity check, and contacts appear normal.

Corrective Action Which Will Be Taken:

1.

Review interim test methods to ensure that continuity testing as a result of this concern is adequate because the only method of visual inspection requires destroying contact.

2.

Temporarily increase the frequency of continuity testing until results justify return to normal frequency.

3.

Long Term - Determine the feasibility of improving the quality of the current switches or replacement by another qualified vendor.
