Dear Mr. Hunter:

This letter provides the final report submitted pursuant to 10CFR50.55(e) concerning in-core thermocouple adaptor cable connectors at Wolf Creek Generating Station (WCGS). This matter was initially reported by Mr. H.K.

Chernoff of Kansas Gas and Electric Company (KG&E) to Mr. Lawrence Martin of the Nuclear Regulatory Commission, Region IV, on August 6,1984.

Subsequent to component testing, more than half of the class lE thermo-couple connectors for the in-core thermocouple adaptor cables were found to have cracked or broken insulation inserts. These connectors were manufactured by CKB Industries, Inc. (Part No. 211140 - male, and Part No. 211141 - female). Although the exact cause cannot be determined, mishandling in the field has been identified as the probable cause of the connector damage observed. As a result of the number of damaged con-nectors discovered, KG&E has elected to replace all of the in-core thermo-couple adaptor cable connectors with new connectors manufactured by CKB Industries, Inc. (Part Numbers 211145 and 211146). These connectors incorpcrate a key and keyway design feature to minimize the possibility of connector damage as a result of mishandling.

The existing in-core thermocouple adaptor cable connectors will be replaced prior to fuel load. Please contact me or Mr. Otto Maynard of my staff if you have any questions concerning this subject.
